The Elite First Aid BleedStop Bandage is an intermediate hemorrhage control system that enables operators to construct an effective pressure dressing for moderate to severe bleeding in locations where a tourniquet is not indicated or is unavailable.

The BleedStop Bandage bridges the operational gap between a basic gauze pad and an all-in-one trauma dressing like the Israeli Bandage. It is designed for steady venous or minor arterial bleeding and is a cost-effective capability for mass casualty response bags, vehicle kits, industrial first aid stations, and Individual First Aid Kits. Within an IFAK, it functions as a critical redundant capability for treating secondary wounds or managing a situation where a primary dressing has failed.

Each field-ready packet is a self-contained system containing four components with distinct roles: a sterile pouch that maintains component integrity until deployment, a non-stick absorbent pad that covers the wound without adhering to the forming clot, a tightly rolled pressure bolus gauze that concentrates focal force directly over the bleeding vessel, and a securing wrap gauze that applies circumferential pressure to lock the dressing in place. Understanding the function of each component before deployment is essential to correct application under stress.

This system carries a critical protocol limitation that all operators must internalize. The BleedStop Bandage is not a tourniquet substitute. Catastrophic, pulsatile, bright-red arterial hemorrhage from an extremity requires a CoTCCC-recommended tourniquet as the immediate intervention. Deploying a pressure dressing in that scenario instead of a tourniquet can result in casualty death.

Field Application Steps

1. SCENE SAFETY AND BSI: Confirm the immediate area is secure before initiating care. Don medical gloves at minimum before making contact with the casualty or the wound.

2. EXPOSE AND ASSESS THE WOUND: Fully expose the injury site to identify the exact source and character of bleeding. Visually confirm the bleed is steady venous or minor arterial flow and does not meet criteria for immediate tourniquet application, such as pulsatile bright-red hemorrhage.

3. APPLY MANUAL DIRECT PRESSURE: Place a gloved hand or available gauze firmly over the wound while you retrieve and prepare the BleedStop packet. Maintain pressure continuously until you are ready to transition to the dressing.

4. INSPECT AND OPEN THE STERILE POUCH: Before opening, inspect the vacuum-sealed pouch for tears, punctures, or water damage. If the pouch is compromised, do not use it. Open only at the indicated notches when you are prepared for immediate application.

5. PREPARE ALL COMPONENTS: Remove the non-stick pad and both gauze rolls from the pouch. Place them on a clean surface or retain them in hand, ready for sequential deployment.

6. APPLY THE NON-STICK PAD: Remove manual pressure just long enough to place the non-stick pad directly over the wound. Ensure the pad covers the entire wound area before proceeding.

7. POSITION THE PRESSURE BOLUS: Place Gauze Roll 1, kept tightly rolled and intact, directly on top of the pad. Center it over the most active point of bleeding to concentrate focal pressure over the bleeding vessel beneath.

8. APPLY THE SECURING WRAP: Begin wrapping Gauze Roll 2 around the limb, starting distal to the wound and progressing proximally toward the torso. Overlap each pass by approximately 50%, fully covering the pad and pressure bolus with each layer.

9. SECURE THE DRESSING: Once the full roll is applied, secure the loose end by tucking it under a previous wrap or using medical tape. The dressing must be tight enough to control bleeding but must not function as a tourniquet, so check distal pulse immediately after application.

10. ASSESS AND MONITOR: Check for a distal pulse at the foot or wrist to confirm circulation has not been fully occluded. Monitor the dressing continuously for bleed-through.

11. MANAGE BLEED-THROUGH IF IT OCCURS: Do not remove the original dressing if blood saturates through it. Apply a second dressing or hemostatic agent directly over the first, apply additional pressure, and reassess the need for tourniquet application.

12. TREAT FOR SHOCK AND INITIATE EVACUATION: Monitor the casualty's vital signs, treat for shock as indicated, and prepare for evacuation to definitive medical care without delay.
