Airway and respiration management components include a Hyfin Vent Chest Seal twin pack for penetrating thoracic trauma, a 14-gauge decompression needle restricted to certified medical professionals, and a 28F nasopharyngeal airway with lubricant for unconscious or semi-conscious casualties with upper airway obstruction. The vented chest seals feature a three-channel design that allows air to escape on exhalation while preventing atmospheric air from entering, directly mitigating tension pneumothorax risk.

The kit deploys via a rip-away Velcro pouch compatible with MOLLE-equipped tactical vests and vehicle headrest mounts, enabling immediate access under duress. Readiness requires monthly inspection of sealed components, expiration dates, tourniquet staging, and pouch integrity. Field Application Steps

1. ESTABLISH SCENE SAFETY: Confirm the immediate threat is neutralized before initiating any medical intervention. Transmit via radio: officer down, your location, nature of injury, and current threat status.

2. DON NITRILE GLOVES: Put on nitrile gloves before making any physical contact with the casualty. This is your first action and is non-negotiable as the primary body substance isolation step.

3. DEPLOY THE KIT: Grasp the pouch handle, unclip the security buckle, and rip the pouch away from the Velcro panel on the vest or headrest mount.

4. APPLY CAT TOURNIQUET FOR EXTREMITY HEMORRHAGE: Place the NAR CAT Gen7 tourniquet high and tight on the injured limb, directly over the uniform. Tighten the windlass rod until bleeding stops completely, secure the rod in the clip, fasten the time strap, and write the time of application on the time strap using the mini marker.

5. ASSESS AIRWAY AND LEVEL OF CONSCIOUSNESS: Check the casualty's level of consciousness and listen for snoring or obstructed respirations. If the casualty is unconscious or semi-conscious and you are trained and authorized, insert the nasopharyngeal airway with lubricant through a nostril into the pharynx.

6. EXPOSE AND INSPECT THE TORSO: Use EMT shears to cut away clothing and fully expose the torso. Inspect the front, back, and sides for penetrating chest wounds.

7. APPLY HYFIN VENT CHEST SEAL: If a penetrating chest wound is identified, apply one Hyfin Vent Chest Seal over the entry wound as the casualty exhales. Apply the second seal to any confirmed exit wound and monitor continuously for signs of tension pneumothorax.

8. REASSESS HEMORRHAGE CONTROL AND PERFORM BLOOD SWEEP: Re-check the tourniquet. If bleeding persists, apply a second tourniquet directly proximal to the first. Conduct a rapid blood sweep of the entire body to identify any additional wounds.

9. PACK JUNCTIONAL OR NON-TOURNIQUETABLE WOUNDS: Pack any wounds in junctional zones such as the groin or axilla with QuikClot Combat Gauze, applying a minimum of 3 minutes of continuous, firm direct pressure. Secure the packed wound with the Israeli Emergency Bandage to maintain pressure.

10. PREPARE FOR EVACUATION: Keep the casualty warm to prevent hypothermia regardless of ambient temperature. Communicate all interventions performed, including tourniquet time, to responding EMS or law enforcement units, and monitor continuously until handoff to a higher level of care.
