Deployment protocol follows a structured sequence aligned with MARCH priorities: scene safety and BSI first, followed by hemorrhage control, airway and respiration assessment, circulation and shock monitoring, hypothermia prevention, and continuous reassessment pending EMS handoff. Tourniquet application time must be documented, and all interventions must be reported verbally to incoming EMS. Personnel must operate strictly within their certified scope of practice, as several components including suture sets, hemostats, and oral airways require formal clinical training to deploy safely.

Sustained readiness requires quarterly inspection of all sterile components for expiration and package integrity, hardware function checks on the tourniquet windlass and penlight, and full inventory restocking after any deployment. The recommended staging protocol places a tourniquet, shears, and gloves on the exterior MOLLE webbing for access within the first thirty seconds of an incident.

Field Application Steps

1. ESTABLISH SCENE SAFETY AND BSI: Verify the scene is safe, confirm the firearm is cleared and the range is cold, then don nitrile gloves before making any contact with the casualty. Stage the kit open next to the casualty and establish verbal contact to identify injury locations.

2. CONTROL MASSIVE HEMORRHAGE: Identify the source of pulsatile bleeding and prioritize it above all other interventions. Apply the windlass tourniquet high and tight on the injured limb, well above the wound, and tighten the windlass until all bleeding stops completely. Record the exact time of application.

3. ASSESS AND MANAGE AIRWAY AND RESPIRATION: Confirm airway patency. If the casualty is talking, the airway is currently open and no adjunct is needed. Use utility shears to cut away clothing over the chest, identify any penetrating wound, wipe it clean, and apply a vented chest seal centered directly over the wound. Log-roll the casualty to check for an exit wound and apply a second chest seal if one is found.

4. ASSESS CIRCULATION AND MONITOR FOR SHOCK: Check for a rapid and weak pulse, assess skin color and temperature as indicators of shock progression. Obtain a baseline blood pressure reading using the BP unit and stethoscope if conditions permit.

5. PREVENT HYPOTHERMIA AND CONDUCT SECONDARY SURVEY: Wrap the casualty in an emergency blanket to prevent heat loss, even in warm ambient conditions, as hypothermia is a component of the trauma triad of death. Perform a rapid head-to-toe assessment for missed injuries. Do not remove the tourniquet or chest seal. Immobilize any identified fractures using the SAM splint formed to the limb and secured with the elastic bandage.

6. MONITOR, REASSESS, AND DOCUMENT: Continuously reassess tourniquet effectiveness and monitor the quality of the casualty's breathing. Watch for signs of developing tension pneumothorax including severe respiratory distress and tracheal deviation. Document all injuries and interventions rendered, including the tourniquet application time, and prepare a complete verbal report for incoming EMS.
