Its components address two distinct wound categories: limb wounds requiring tourniquet application and junctional wounds, such as groin, armpit, and neck injuries, requiring wound packing and sustained direct pressure. The kit is not a comprehensive first aid system and does not include components for minor injuries, airway management, or penetrating thoracic trauma.

The kit contains a windlass tourniquet with time strap, a hemostatic gauze pad impregnated with a clotting-accelerating agent, a Hema-Seal gauze roll for cavity filling, a 5x9 inch trauma pad for external pressure, nitrile gloves for body substance isolation, and a permanent marker for recording tourniquet application time. Each component serves a specific operational function within a defined deployment protocol. Operator familiarity with all components before an incident is a readiness requirement, not a recommendation.

Deployment protocol is organized across three phases: scene management and exposure, hemorrhage control, and casualty management through EMS handoff. The objective standard for tourniquet effectiveness is complete cessation of pulsatile bleeding. Wound packing requires a minimum of three to five minutes of uninterrupted direct pressure. All kit components are single-use and single-patient. Any deployment, including training use, renders the kit non-mission-capable and requires immediate full replacement.

Field Application Steps

1. CONFIRM SCENE SAFETY: Before approaching the casualty, verify the scene is safe to operate in and mitigate any ongoing hazards such as unstable equipment or downed power lines. Do not approach until the threat to the operator is controlled.

2. ACTIVATE EMERGENCY RESPONSE: Direct another bystander to call 911 immediately and provide a clear location and description of the injury. Do not delay this step or assume it has been done.

3. DON PPE: Open the kit and put on the nitrile gloves before making any patient contact. Body substance isolation is the operator's first priority and must not be skipped regardless of urgency.

4. EXPOSE THE WOUND: Remove or cut away clothing to fully visualize the injury site. Do not attempt assessment or treatment through clothing.

5. IDENTIFY BLEEDING TYPE: Determine whether the wound is on a limb (arm or leg) or in a junctional area (groin, armpit, neck). This determination drives the entire hemorrhage control protocol. Proceed to Step 6 for limb bleeding or Step 9 for junctional bleeding.

6. APPLY TOURNIQUET HIGH AND TIGHT: Place the windlass tourniquet 2 to 3 inches above the wound, as close to the torso as possible. Never place the tourniquet directly over a joint such as the knee or elbow.

7. CINCH THE STRAP AND TWIST TO EFFECT: Pull the strap through the friction buckle as tightly as possible, then rotate the windlass rod until pulsatile bleeding ceases completely. Cessation of pulsatile bleeding is the objective standard for adequate application.

8. SECURE THE ROD AND RECORD TIME: Lock the windlass rod in the clip and secure the time strap. Use the permanent marker to write the exact time of tourniquet application on the strap. This information is critical for receiving medical professionals.

9. PACK THE WOUND WITH HEMOSTATIC GAUZE: For junctional wounds where a tourniquet cannot be applied, locate the primary bleeding source within the wound cavity. Feed the hemostatic gauze pad directly into the wound while applying pressure, packing until the cavity is full.

10. FILL REMAINING CAVITY: If space remains in the wound cavity after hemostatic gauze is placed, use the Hema-Seal gauze roll to fill it completely and build internal pressure.

11. APPLY SUSTAINED DIRECT PRESSURE: Apply firm, continuous, two-handed pressure directly over the packed wound for a minimum of 3 to 5 minutes. Do not release pressure to check the wound during this interval.

12. COVER AND HOLD THROUGH EMS HANDOFF: Place the 5x9 inch trauma pad over the packed wound and continue pressure until EMS arrives. At handoff, provide a concise report covering mechanism of injury, time of tourniquet application if applicable, and all interventions performed.
