The kit is deployed upon visual confirmation of indicators including pulsatile or spurting blood, a rapidly spreading pool on the floor, saturated clothing or dressings, traumatic amputation, or failed direct pressure. Calling 911 is the first priority before deploying any component. The kit's scope is strictly limited to arms and legs, and it is not designed for torso, neck, or head wounds. Operators must confirm the wound is on a limb before proceeding.

The kit contains a windlass tourniquet applied high and tight to occlude arterial blood flow, an Israeli Bandage with an integrated pressure bar for focused wound dressing, two 5-by-9-inch trauma pads for direct wound coverage, and an elastic pressure strap to maintain consistent dressing pressure. Supporting components include nitrile gloves for bloodborne pathogen protection, bandage shears for wound exposure, a permanent marker for documenting tourniquet application time, and an emergency blanket to prevent hypothermia, a life-threatening complication of hemorrhagic shock.

Readiness protocol requires quarterly inspection of all sterile packaging integrity, expiration dates, and component presence. The kit is a single-use system, and the entire kit must be fully restocked after any deployment before it returns to a ready state. Storage in a climate-controlled, clearly marked, and personnel-accessible location is required to maintain operational readiness.

Field Application Steps

1. SCENE SAFETY AND PPE: Confirm the area is safe and the hazard is neutralized, for example, verify the machine is powered down. Open the kit and immediately don one pair of nitrile exam gloves before making patient contact.

2. EXPOSE AND ASSESS THE WOUND: Use bandage shears to cut clothing from hip to knee, fully exposing the entire limb. Visually identify the source of the most significant bleeding and confirm it is life-threatening and located on the extremity.

3. APPLY THE TOURNIQUET HIGH AND TIGHT: Route the tourniquet strap around the limb as high up as possible, between the wound and the heart. Feed the strap tip through the buckle and pull aggressively until maximally tight, then secure the strap back on itself with the hook-and-loop fastener.

4. TIGHTEN THE WINDLASS UNTIL BLEEDING STOPS: Twist the windlass rod until bleeding stops completely. Use cessation of bleeding as the indicator, not patient pain level, then lock the rod securely into the C-clip.

5. DOCUMENT TOURNIQUET APPLICATION TIME: Write the current time on the tourniquet time strap using the permanent marker, for example, "14:30." If the time strap is unavailable, write the time directly on the patient's forehead. Do not cover the tourniquet; keep it visible for arriving EMS.

6. APPLY SECONDARY WOUND DRESSING: For wounds not requiring a tourniquet, or for secondary wound sites, place a trauma pad directly over the bleeding site. Apply the Israeli Bandage over the pad, reversing wrap direction through the integrated pressure bar to apply focused pressure, then secure with the closure clip.

7. REINFORCE IF BLEEDING CONTINUES: If blood soaks through the first trauma pad, apply the second trauma pad directly on top of the first. Do not remove the saturated pad. Wrap the elastic pressure strap over both pads, overlapping each pass by at least 50 percent to maintain consistent, even pressure.

8. PREVENT SHOCK AND MONITOR THE PATIENT: Lay the patient flat and elevate the legs if possible. Unfold the emergency blanket and wrap it fully around the patient to prevent heat loss, then reassure the patient and continuously monitor level of consciousness and tourniquet tightness.

9. EXECUTE HANDOFF TO EMS: Provide a clear, concise verbal report to arriving EMS covering the injury, the time of tourniquet application, current bleeding status, and actions taken. Transfer care and step back from the patient.
