Butterfly wound closures are indicated for simple, linear lacerations with clean, straight edges that can be brought together without excessive tension. Ideal deployment sites include fingers, knuckles, and foreheads, where moderate skin tension allows the device to maintain reliable approximation. The closure works by anchoring adhesive wings to the skin on either side of the wound, physically pulling the edges together and holding them in position throughout the healing cycle. This mechanical support reduces scarring potential and supports the body's natural repair process without professional intervention.

The device consists of four functional components: adhesive wings that anchor to the skin under tension, a flexible elastic body that conforms to contours and accommodates minor movement, a non-adherent center pad that protects the wound bed from adhesive contact, and an individual sterile pouch that maintains readiness until the point of use. Pouch integrity must be confirmed before deployment. The non-adherent pad is a critical protective element, enabling atraumatic removal without disturbing healing tissue.

Operators must assess wound suitability before application. Butterfly closures are contraindicated for wounds with active arterial bleeding, deep involvement of muscle or bone, jagged or irregular edges, high infection risk such as animal bites, heavy contamination, or placement directly over a joint where tension would immediately compromise the closure.

Field Application Steps

1. ASSESS WOUND SUITABILITY: Confirm the laceration is minor, linear, and has clean edges that can be approximated without excessive tension. If the wound involves deep tissue, active pulsatile bleeding, jagged edges, contamination, or is located directly over a joint, do not proceed and seek professional medical care.

2. ACHIEVE HEMOSTASIS: Apply direct pressure to the wound until active bleeding has slowed or stopped. Butterfly closures cannot be applied effectively to an actively bleeding wound and will not adhere properly to wet or blood-saturated skin.

3. CLEAN AND DRY THE WOUND SITE: Irrigate the wound and surrounding skin to remove surface debris. Thoroughly dry the skin on both sides of the laceration before opening the device. Adhesive performance depends on clean, dry contact surfaces.

4. VERIFY STERILE POUCH INTEGRITY: Before opening, inspect the individual sterile pouch for tears, punctures, or compromised seals. Do not use any closure with a damaged or questionable package.

5. OPEN THE STERILE POUCH: Peel open the pouch using clean or gloved hands, taking care not to contaminate the device. Handle the closure by its edges to preserve sterility of the adhesive wings and center pad.

6. ANCHOR THE FIRST WING: Peel the backing from one adhesive wing and press it firmly to the skin on one side of the laceration, placing it perpendicular to the wound line. Ensure the non-adherent center pad is aligned to sit directly over the wound.

7. APPROXIMATE THE WOUND EDGES: Using your free hand, gently push the skin on the opposite side of the laceration toward the first anchor point, bringing the wound edges into close contact. The edges should align evenly without overlap or inversion.

8. ANCHOR THE SECOND WING UNDER TENSION: While maintaining wound approximation, peel the backing from the second adhesive wing and press it firmly to the skin on the opposite side, pulling it taut to maintain closure tension. Confirm both edges remain closely approximated beneath the center pad.

9. APPLY ADDITIONAL CLOSURES AS NEEDED: For lacerations longer than the span of a single closure, apply additional butterfly closures in parallel, spacing them evenly along the wound length to distribute tension and maintain uniform approximation.

10. MONITOR FOR COMPLICATIONS: Check the wound periodically for signs of infection including increasing redness, warmth, swelling, discharge, or odor. If any of these signs develop, or if the closure fails to maintain approximation, seek professional medical evaluation.
