The Mayday Multiperson Trauma Medical Unit is a 100-person mass casualty incident response system designed to provide point-of-injury hemorrhage control, fracture management, and shock treatment for trained lay responders operating a Casualty Collection Point until professional EMS assumes command.

The kit is organized into seven functional supply groups covering massive hemorrhage control, musculoskeletal injury, minor wound and burn care, decontamination, environmental handling, assessment support, and personal protective equipment. Primary hemorrhage control capability is built around 25 Blood Stopper Trauma Dressings, 20 Multi Trauma Dressings, 10 ACE bandages, and 15 rolls of surgical adhesive tape. Operators must understand that commercial tourniquets, hemostatic agents, chest seals, and airway adjuncts are intentionally excluded from this kit, placing those injury categories outside its scope.

Deployment follows a four-phase protocol: Staging and Setup, Triage and Initial Intervention, Secondary Survey and Treatment, and Handoff to EMS. During triage, operators prioritize life-threatening external hemorrhage first, instruct conscious casualties to self-apply direct pressure, then deploy trauma dressings for severe bleeds without delay. Casualties with significant trauma are treated for shock by positioning them flat and covering them with a Paramedic or Solar Blanket. No food or fluids are administered.

Readiness requires quarterly inventory inspections for damaged or missing items and annual checks of all expiration dates on sterile supplies. After any deployment, including partial use, the unit must be immediately removed from service, fully inventoried, and completely resupplied before returning to its ready-storage location. A partially stocked unit is never acceptable for return to active readiness status.

Field Application Steps

1. CONFIRM SCENE SECURITY: Verbally confirm with law enforcement or a designated safety officer that the Casualty Collection Point location is secure before retrieving or opening the unit.

2. RETRIEVE AND POSITION THE UNIT: Move the Mayday Trauma Medical Unit duffel bag to the center of the CCP and place it on a clean, dry surface. Open the duffel completely.

3. DON PPE IMMEDIATELY: Put on a pair of nitrile gloves before any casualty contact. Keep the glove supply accessible at all times and change to a fresh pair for each new casualty treated.

4. ORGANIZE SUPPLIES BY FUNCTION: Separate contents into three working groups: Massive Hemorrhage, Musculoskeletal, and Minor Wounds. Pre-organization at this stage directly reduces treatment delays when casualties arrive.

5. TRIAGE BY BLEEDING PRIORITY: Conduct a rapid visual sort of all casualties at the CCP. Identify and move immediately to address any massive, life-threatening external hemorrhage before all other injuries.

6. DIRECT CONSCIOUS CASUALTIES TO SELF-TREAT: Instruct conscious casualties with moderate bleeding to apply firm, direct pressure to their wounds using available cloth or their own hands while you address critical cases.

7. DEPLOY HEMORRHAGE CONTROL DRESSINGS: Apply a Blood Stopper Trauma Dressing directly to severe bleeding wounds. For larger wound surfaces, place a Multi Trauma Dressing and secure it tightly with an ACE bandage. Do not delay this step.

8. MANAGE FRACTURES AND MUSCULOSKELETAL INJURIES: Once life-threatening bleeding is controlled, use the 18-inch or 24-inch splint kits to immobilize deformed limbs. Use triangle bandages to create slings for arm injuries.

9. REDIRECT WALKING WOUNDED: Direct ambulatory casualties to a separate area within the CCP. Assign one operator to manage this group using adhesive bandages, antiseptic towelettes, and gauze from the minor wound supplies.

10. TREAT FOR SHOCK: Lay any casualty with significant trauma flat after bleeding is controlled and cover them with a Paramedic or Solar Blanket. Do not give the casualty anything to eat or drink.

11. APPLY ICE PACKS TO CLOSED INJURIES: Activate instant ice packs and apply them to sprains, strains, or contusions. Always wrap the pack in a cloth or triangle bandage before placing it against skin.

12. EXECUTE EMS HANDOFF USING M.I.S.T. FORMAT: When EMS arrives, provide a concise report for each critical casualty covering Mechanism of Injury, Injuries Found, Signs observed, and Treatments Given. Be prepared to report the total casualty count and injury types across the CCP.
