Quick Overview

The R&B Bariatric Transfer Sheet with Backboard Sleeve (Model 181YL-BB) is a hybrid patient movement device used by fire-rescue, EMS, and transport personnel to safely transfer bariatric patients up to 700 lbs through confined spaces where standard litters cannot operate.

The device combines a 60" x 80" double-layer nylon-reinforced vinyl chassis with an integrated backboard sleeve that accepts standard 16" or 18" backboards. Without the backboard inserted, the sheet functions as a standard soft transfer surface and will exhibit significant flexion under bariatric load, a condition known as the "taco effect." Inserting a compatible backboard transforms the assembly into a rigid transfer platform, maintaining spinal alignment and distributing patient weight across the centralized webbing system to all ten Box-X stitched handholds. This hybrid design is the operational advantage that distinguishes it from standard soft sheets in bariatric deployment scenarios.

Deployment requires a minimum of four trained providers, with six to eight recommended for patients approaching the upper weight limit. A designated Team Leader is mandatory to coordinate lift commands, correct provider technique before any lift is initiated, and provide verbal guidance during transport. All providers must lift from legs and core, never the back, and the device must never be dragged, as friction contact with surfaces compromises the vinyl barrier and structural integrity.

The impervious vinyl surface supports post-mission decontamination using hospital-grade disinfectants, making the device suitable for transfers involving significant bodily fluid exposure. Post-mission protocol requires backboard removal from the sleeve, full surface wipe-down with approved virucide, complete air drying before storage, and return to the dedicated nylon storage bag away from direct sunlight and extreme temperatures.

Field Application Steps

1. PRE-DEPLOYMENT INSPECTION: Pull firmly on each of the ten webbing handholds and check for fraying, tears, or Box-X stitch separation. Inspect the vinyl surface for cuts or punctures, check all seams including the backboard sleeve perimeter, and confirm the device is clean and free of contaminants from prior use. If any damage is found, remove the device from service immediately.

2. PREPARATION AND STAGING: Confirm the patient's weight does not exceed the 700 lb operational limit. Assign a minimum of four providers, preferably six for bariatric patients, and designate one Team Leader. Insert a compatible 16" or 18" wide backboard fully into the underside sleeve, then stage the assembled device adjacent to the patient.

3. PATIENT POSITIONING: Log-roll the patient away from the sheet using coordinated provider movement to maintain spinal alignment throughout the roll. Position the sheet and backboard assembly flat against the patient's back, then log-roll the patient back onto the center of the sheet, aligning the patient's center of mass directly over the backboard.

4. HANDHOLD ASSIGNMENT: Assign each provider to specific handholds on the longitudinal sides of the sheet. Providers grasp assigned handholds using an underhand grip, palms facing up, to maximize grip strength and load control.

5. LIFT EXECUTION: The Team Leader issues the command sequence: "Lifting on three. One... Two... THREE!" All providers lift simultaneously, driving from legs and core with backs straight. No provider should initiate or adjust the lift independent of the Team Leader's command.

6. TRANSPORT TO DESTINATION: Move toward the destination in a coordinated, controlled fashion with the Team Leader providing active verbal guidance around obstacles and through confined spaces such as hallways or stairwells. Maintain synchronized movement and consistent lift height throughout transport.

7. PATIENT TRANSFER TO COT: Position the loaded sheet directly over the ambulance cot or receiving surface. Lower the patient in a controlled, coordinated manner on the Team Leader's command. No sudden drops are permitted.

8. POST-MISSION DECONTAMINATION AND STORAGE: Remove the patient from the sheet using a slide or log-roll technique, then remove the backboard from the sleeve. Wipe all vinyl surfaces, handholds, and seams thoroughly with an approved hospital-grade disinfectant or virucide. Allow the sheet to air dry completely, then fold, return to the storage bag, and store in a clean, dry apparatus compartment away from direct sunlight, extreme temperatures, and sharp objects.
