The Rothco Fast Access Tactical Trauma Kit is a point-of-injury trauma system designed for trained law enforcement officers, EMS personnel, and tactical operators to manage life-threatening hemorrhage and airway compromise until definitive care is available.

The kit's components are organized around the M-A-R-C-H algorithm, addressing Massive Hemorrhage, Airway, Respiration, Circulation, and Head Injury/Hypothermia in sequence. Hemorrhage control capability includes a rubber tourniquet, BleedStop hemostatic bandages, a 12"x30" multi-trauma dressing, three 5"x9" abdominal pads, and elastic and gauze bandages for pressure dressing construction. Surgical and diagnostic tools include EMT shears, a hemostat, a sutures package, and stainless steel tweezers. The Convertipack carry platform deploys as a standalone shoulder or waist pack and integrates with existing gear via MOLLE attachment.

Operators must be aware of two critical capability gaps in this kit. It contains no occlusive dressings for penetrating chest trauma and no needle decompression catheter for tension pneumothorax, both of which are lethal thoracic injuries common in tactical environments. The manual explicitly directs operators to supplement with at least two vented chest seals and a needle decompression device before deploying this kit in any tactical trauma role.

The rubber tourniquet and BleedStop bandage also carry documented limitations. The rubber tourniquet is harder to self-apply and less effective than CoTCCC-recommended windlass-style tourniquets such as the C-A-T or SOF-T Wide. The BleedStop bandage is a basic hemostatic and is not equivalent to military-grade options such as QuikClot or Celox for high-pressure arterial or deep cavitational bleeding. Operators should treat this kit as a foundation and supplement accordingly.

Field Application Steps

1. CONFIRM SCENE SAFETY AND DON PPE: Verify no fire risk and that traffic or threats are controlled before approaching the casualty. Don latex gloves from the kit before any patient contact to maintain BSI protection for both the operator and the casualty.

2. DIRECT PRESSURE AND KIT ACCESS: Verbally direct the casualty or a bystander to apply direct pressure to the wound site while you access and open the kit. This maintains hemorrhage control while you prepare your tools.

3. EXPOSE THE INJURY: Use the EMT Shears to cut the casualty's clothing away from the wound, cutting from cuff to hip for a thigh injury. Full exposure is required to accurately assess the nature and severity of bleeding.

4. APPLY TOURNIQUET FOR CATASTROPHIC ARTERIAL HEMORRHAGE: If pulsating arterial bleeding is identified on a limb, apply the rubber tourniquet as high as possible on the injured limb and secure tightly. Record the exact time of tourniquet application immediately, as this information is critical for incoming EMS.

5. ASSESS AND MANAGE THE AIRWAY: Check whether the casualty is responsive. If the casualty is conscious and responsive, the airway is patent and the oral airway device is not indicated. If the casualty is unconscious and airway compromise is present, trained personnel may insert the oral airway adjunct.

6. CONTROL SECONDARY HEMORRHAGE: Perform a rapid full-body check for additional bleeding sources. For secondary lacerations, apply a sterile 4"x4" gauze pad directly to the wound and secure with the 2" elastic bandage to create a pressure dressing.

7. ADDRESS JUNCTIONAL WOUNDS: For wounds in areas where a limb tourniquet cannot be applied, such as the groin or axilla, apply a BleedStop hemostatic bandage directly to the wound with sustained firm pressure. Note that BleedStop is a basic hemostatic and may be insufficient for high-pressure arterial bleeding.

8. PREVENT HYPOTHERMIA: Minimize the casualty's exposure to the environment by covering them with any available material to retain body heat. Hypothermia in a hemorrhagic casualty accelerates coagulopathy and worsens survivability.

9. REASSESS ALL INTERVENTIONS: Confirm tourniquet tightness and proper positioning. Verify all dressings are secure and hemorrhage is controlled. If bleeding continues through a dressing, do not remove it; apply additional dressing material on top and increase pressure.

10. PREPARE FOR HANDOFF AND EVACUATION: Package remaining supplies and prepare a verbal handoff for incoming EMS. Report tourniquet application time, all treatments rendered, and the casualty's current level of responsiveness.
