A bleeding control treatment pack is a field-ready assembly of hemorrhage control tools used by trained operators to stop life-threatening external bleeding from extremities and junctional regions at the point of injury before EMS arrives.

The Luminary SKU 91166 Bleeding Control Treatment Pack is built around two distinct intervention protocols. Protocol A addresses life-threatening extremity bleeding through mechanical occlusion using a windlass tourniquet applied high and tight above the wound site. Protocol B covers junctional and non-tourniquetable wounds using hemostatic blood-clotting gauze packed directly onto the bleeding source, followed by sustained direct pressure and a conforming pressure dressing. The five-step deployment sequence, covering scene safety, injury assessment, hemorrhage control, dressing application, and casualty handoff, must be followed in order at every deployment without exception.

The pack's component groups are organized by function to support fast, sequential deployment. Group 1 provides mechanical hemorrhage control through a primary windlass tourniquet and a backup strap-and-buckle tourniquet for secondary application. Group 2 provides junctional and compressible wound coverage through hemostatic gauze, a Hema-Seal trauma pad with conforming gauze roll, three 5x9 trauma pads, and sterile gauze pads. Group 3 establishes operator protection through nitrile exam gloves, which are the first item deployed before any patient contact, and first aid tape to secure finished dressings.

This pack is designed for a single casualty and a single injury. It is not a comprehensive trauma system and carries no capability for airway or breathing emergencies. Readiness depends entirely on operator certification, correct pre-deployment inspection on a quarterly basis, and adherence to the single-use replenishment policy that prohibits returning any partially depleted pack to service.

Field Application Steps

1. ESTABLISH SCENE SAFETY AND BSI: Confirm the scene is safe to enter before making patient contact. Don both nitrile exam gloves immediately to establish Body Substance Isolation and protect yourself and the casualty from fluid-borne pathogens.

2. ASSESS THE INJURY: Identify the location, severity, and type of bleeding. Determine whether the wound is on an extremity, making it eligible for tourniquet application, or in a junctional region such as the neck, shoulder, or groin, requiring wound packing under Protocol B.

3. APPLY TOURNIQUET FOR EXTREMITY BLEEDING (PROTOCOL A): Place the windlass tourniquet high and tight on the injured limb, above the wound and as close to the torso as possible. Pull the strap through the friction buckle removing all slack, then twist the windlass rod until bleeding has completely stopped and lock the rod in the windlass clip.

4. RECORD TOURNIQUET TIME: Write the exact time of tourniquet application on the time strap using a permanent marker if available. Do not cover the tourniquet at any point.

5. PACK JUNCTIONAL WOUNDS (PROTOCOL B): Identify the point of most significant bleeding within the wound cavity and apply the hemostatic blood-clotting gauze pad directly onto that point. Pack the remainder of the wound cavity firmly with 5x9 trauma pads or conforming gauze roll until the cavity is full.

6. APPLY DIRECT PRESSURE: Press firmly with both hands directly over the packed wound and hold continuous pressure for a minimum of 3 to 5 minutes without lifting to assess.

7. APPLY PRESSURE DRESSING: Place the Hema-Seal 8x10 trauma pad over the packed wound, then wrap the 4-inch conforming gauze roll tightly around the limb with pressure focused directly over the wound site. Secure the end of the roll with first aid tape.

8. MANAGE SATURATED DRESSINGS: If the dressing becomes saturated, apply additional dressings directly on top. Do not remove saturated dressings, as removal disrupts clot formation.

9. ADDRESS TOURNIQUET FAILURE: If the applied windlass tourniquet does not stop bleeding, remove all remaining slack and apply the backup tourniquet directly beside the first tourniquet.

10. REASSESS AND PREPARE FOR HANDOFF: Monitor the casualty continuously, keep them warm to prevent hypothermia, and treat for shock. Communicate all interventions to incoming EMS personnel, including the exact time of tourniquet application and all materials used.
