Quick Overview

The Luminary Global Quick-Tight Windlass Tourniquet is a field-deployable hemorrhage control device used by operators, first responders, and prepared civilians to achieve complete arterial occlusion in a life-threatening extremity bleed when direct pressure has failed.

The Quick-Tight Windlass Tourniquet is designed for deployment in scenarios where uncontrolled arterial hemorrhage from an arm or leg presents an immediate threat to life. Its five-component system, consisting of a high-performance nylon strap, D-Clasp buckle, single-ring windlass mount, secure grip windlass rod, and windlass clip, works in concert to apply and lock circumferential pressure sufficient to stop arterial blood flow. The D-Clasp design allows application without sliding over a hand or foot, a critical capability in constrained or high-stress environments.

Correct placement is the foundation of effective deployment. The strap must be positioned 2 to 3 inches proximal to the wound site, never over a joint or atop bulky objects. Once the strap is pulled fully taut and secured via its hook-and-loop fastener, the windlass rod is twisted until hemorrhage stops and the distal pulse is no longer palpable. The rod is then locked into the windlass clip to prevent pressure release during patient movement and transport.

Post-application protocol is as critical as the application itself. Application time must be recorded on the tourniquet or directly on the patient's skin using a permanent marker. The device must remain uncovered and visible to all subsequent care providers. Once applied, the tourniquet is removed only by qualified medical personnel in a clinical setting, as premature removal can result in rapid re-bleeding, profound shock, and death.

Field Application Steps

1. CONFIRM LIFE-THREATENING HEMORRHAGE: Verify the presence of pulsatile or large-volume arterial bleeding from an extremity that is not controlled by direct pressure. Expose the limb to identify the injury site, or apply over clothing if exposure is too time-consuming.

2. POSITION THE TOURNIQUET: Place the tourniquet strap 2 to 3 inches proximal to (above) the wound site. Never place the device directly over a joint such as the knee or elbow, and ensure no bulky items are underneath the strap.

3. ROUTE AND SECURE THE STRAP: Thread the free end of the nylon strap through the outer slot of the D-Clasp. Pull the strap aggressively to eliminate all slack, then secure it back on itself using the hook-and-loop fastening system. The tourniquet must be as tight as possible before engaging the windlass.

4. APPLY WINDLASS PRESSURE: Grasp the windlass rod and turn it to progressively shorten the strap loop. Continue turning until hemorrhage is completely controlled. If conditions permit, continue until the distal pulse is no longer palpable, confirming total arterial occlusion. Do not stop tightening due to patient pain.

5. LOCK THE WINDLASS ROD: Secure the windlass rod into the C-shaped windlass clip to prevent any unwinding during patient movement or transport. Confirm the rod is fully seated and will not release under pressure.

6. DOCUMENT APPLICATION TIME: Write the time of tourniquet application on the device or directly on the patient's skin using a permanent marker, for example "T-14:30." This information is critical for all subsequent care providers managing tourniquet duration.

7. KEEP THE TOURNIQUET VISIBLE: Do not cover the tourniquet with clothing, bandages, or any other material. It must remain fully visible to every medical provider who assumes care of the patient.

8. REASSESS AND RESPOND: Monitor for continued or restarting hemorrhage. If bleeding resumes and the tourniquet is confirmed tight, apply a second tourniquet directly proximal to the first. Do not remove the original device.
