Field Application Steps

1. SCENE SAFETY AND BSI: Confirm the scene is clear of environmental hazards including unstable equipment and electrical risk. Don High Risk Nitrile Gloves before any patient contact and stage the kit in an accessible position for rapid component retrieval.

2. PRIMARY ASSESSMENT AND C-SPINE CONTROL: Introduce yourself to the patient and direct them to self-immobilize the head and neck if the mechanism of injury warrants spinal precaution. Conduct a rapid MARCH/ABCDE assessment to identify all life-threatening conditions before initiating treatment.

3. HEMORRHAGE CONTROL: Identify and prioritize the most significant bleeding wound. Apply multiple 4-inch by 4-inch sterile gauze pads directly to the wound site and secure firmly with a 4-inch sterile gauze roll to establish and maintain direct pressure.

4. DIAGNOSTIC ASSESSMENT: Once active bleeding is controlled, obtain baseline vitals using the sphygmomanometer and stethoscope to evaluate perfusion and identify signs of shock. Use the doctor's pen light to assess bilateral pupillary response for indicators of neurological compromise or head injury.

5. SPLINTING AND SECONDARY INJURY MANAGEMENT: Cut away clothing from the injury site using the Lister bandage scissors to expose the deformity. Pad and position the appropriate splint, either the 15-inch padded board splint or the 4.25-inch by 36-inch flexible splint, and secure it using triangular bandages or gauze rolls, immobilizing the joints both above and below the fracture site.

6. PAIN AND SHOCK MANAGEMENT: Apply a cold pack directly to the splinted extremity to reduce swelling and pain through vasoconstriction. Cover the patient with the emergency Mylar blanket to prevent hypothermia and continuously reassess vitals and the effectiveness of all interventions throughout this phase.

7. DOCUMENTATION AND TRANSFER OF CARE: Record all identified injuries, baseline and interval vitals, and every intervention with accurate timestamps. Deliver a concise hand-off report to transporting EMS and secure all contaminated materials in the biohazard bag before clearing the scene.
