The Guardian Bleeding Control Kit (IFAK) is a point-of-injury hemorrhage control system designed for trained bystanders and initial responders to stop life-threatening blood loss and stabilize a casualty during the critical interval before Emergency Medical Services arrive.

Catastrophic blood loss is the leading cause of preventable death in traumatic injuries. This kit addresses that threat directly through three primary intervention pathways: tourniquet application for extremity hemorrhage, wound packing for junctional bleeding in areas such as the neck, groin, and axilla, and pressure dressing application for deep lacerations. Every component serves a defined operational role, and the deployment protocol is sequenced to match the speed and pressure of a real incident.

The kit's component load reflects field-tested priorities. The SOF Tourniquet Gen 4 provides circumferential arterial occlusion via a windlass and tri-ring system. Two rolls of Compressed Krinkle Gauze enable deep-cavity wound packing where tourniquet application is not anatomically viable. The Super Stop Bandage secures packed wounds under sustained pressure. Nitrile gloves, an emergency survival blanket, utility shears, a permanent marker, and a high-visibility red MOLLE carrier complete the operator's capability set.

Readiness is not a one-time action. Monthly inspection is the standard protocol, covering packaging integrity, tourniquet pre-staging, shear function, and full contents inventory. This kit is a single-use item. Any component deployed on a casualty is considered contaminated and must be replaced before the kit is returned to a mission-ready state.

Field Application Steps

1. CONFIRM SCENE SAFETY AND DON PPE: Verify all immediate threats are mitigated before approaching the casualty. Open the red MOLLE bag and put on one pair of nitrile gloves before initiating any patient contact.

2. EXPOSE AND IDENTIFY THE BLEEDING SOURCE: Locate the most severe, life-threatening wound. Use the 7-1/4 inch utility shears to cut away clothing, webbing, or gear obscuring the injury site and fully expose the wound.

3. SELECT THE CORRECT INTERVENTION: Assess wound location to determine the appropriate protocol. A limb wound on an arm or leg requires tourniquet application. A wound in a junctional area such as the neck, axilla, or groin, or a deep packable cavity, requires wound packing and pressure dressing.

4. APPLY THE TOURNIQUET FOR LIMB HEMORRHAGE: Route the SOF Tourniquet strap around the limb 2 to 3 inches above the wound. If wound location is unclear, go high and tight. Feed the strap through the Tri-Ring, pull all slack, and turn the windlass rod until bright red pulsating bleeding stops completely. Secure the windlass in the clip and engage the safety screw if time permits.

5. DOCUMENT TOURNIQUET APPLICATION TIME: Using the permanent marker, write the exact time of tourniquet application on the tourniquet's time strap. This information is critical for receiving medical professionals and must not be skipped.

6. PACK THE WOUND FOR JUNCTIONAL HEMORRHAGE: Open a package of Compressed Krinkle Gauze and push the leading end as deep as possible into the wound cavity, directly onto the suspected bleeding source. Continuously feed gauze in until the cavity is tightly packed, then apply firm direct pressure with both hands for a minimum of 3 minutes.

7. APPLY THE PRESSURE DRESSING: Open the Super Stop Bandage and place the integrated pressure block directly over the packed wound. Wrap the elastic leader tightly around the injury site and secure it with the closing bar to maintain hemostasis.

8. PREVENT SHOCK AND REASSESS: Unfold the emergency survival blanket and cover the casualty to reflect body heat, keeping the face clear. Reassess the tourniquet for tightness and check the dressing for blood saturation. If bleeding is not controlled, re-apply or place a second tourniquet proximal to the first.

9. PREPARE FOR EMS HANDOVER: Communicate all identified injuries and interventions to arriving EMS personnel. Lead with tourniquet application time and location. Transfer care with a complete verbal report to support continuity of clinical treatment.
