The 8-inch Israeli Emergency Abdominal Bandage is a field-deployable hemorrhage control device used by first responders and trained operators to manage large-area, high-volume bleeding wounds to the torso, junctional areas, and amputation stumps where standard compression bandages are insufficient.

This device consolidates four distinct functions into a single deployable unit: primary dressing, pressure applicator, secondary dressing, and closure system. The 12-by-12-inch non-adherent sterile pad protects wound tissue and exposed organs without adhering to them, while the 8-inch elastic leader wrap, extending 13.5 feet when expanded, provides sufficient length for multiple wraps around an adult torso or large limb stump. The integrated Pressure Applicator Bar acts as a force multiplier, focusing up to 30 pounds of direct, localized pressure onto the wound pad when the elastic wrap is routed through it and reversed in direction.

Primary applications include abdominal and trunk trauma, abdominal evisceration, traumatic amputations used in conjunction with a tourniquet, and large junctional wounds at the groin or axilla where tourniquet application is impractical. The device is not a substitute for a TCCC-approved limb tourniquet in cases of catastrophic extremity hemorrhage where tourniquet application is indicated and feasible. Operators must reassess distal circulation after application to the limb to confirm the bandage has not created an unintentional tourniquet effect.

Readiness protocols require periodic inspection of the vacuum-sealed outer packaging for punctures, tears, or loss of seal. A field-ready package will be rigid and flat. Any unit with soft or air-compromised packaging, or any unit past its printed expiration date, must be removed from operational rotation immediately and replaced. This is a single-use device and must be disposed of per biohazard protocols following deployment.

Field Application Steps

1. SCENE SAFETY AND EXPOSURE: Ensure scene safety and don appropriate BSI, including gloves and eye protection. Cut away clothing to fully expose the wound and visually identify the primary source of bleeding.

2. PACKAGE OPENING: Tear open the outer vacuum packaging at the designated tear notches using a firm grip, even with gloved or bloody hands. Remove the inner-wrapped bandage and unroll the elastic leader until the sterile pad is fully exposed.

3. PAD PLACEMENT: Do not touch the sterile surface of the pad. Place the pad directly onto the wound, ensuring it completely covers the entire injury area.

4. PRESSURE APPLICATION: Hold the pad in place with one hand. With the other hand, route the elastic leader over the pad and around the limb or torso, then guide it through the opening of the Pressure Applicator Bar. Pull the elastic leader back in the opposite direction to force the bar firmly onto the wound pad. A significant reduction in bleeding should be observed at this point.

5. SECURING THE DRESSING: Maintain tension on the elastic wrap and continue wrapping over the Pressure Bar and around the limb or torso, covering the entire pad. Each wrap must be tight and overlap the previous layer.

6. CLOSURE: Engage the hooks on the Closure Bar into the bandage layers to lock the dressing in place. Confirm the closure is seated and will not slip. No tape, tucking, or knots are required or recommended.

7. EVISCERATION PROTOCOL (IF APPLICABLE): If abdominal evisceration is present, do not attempt to push exposed organs back into the abdominal cavity. Moisten the sterile pad with sterile water if available, then apply it directly over the exposed organs to protect them from desiccation and contamination before wrapping.

8. POST-APPLICATION REASSESSMENT: Check the edges of the dressing for continued bleeding. If the bandage has been applied to an extremity, assess distal circulation by checking pulse, skin color, and temperature when tactically appropriate. Keep the patient warm to prevent hypothermia and prepare for casualty evacuation.
