The Israeli Bandage integrates four distinct capabilities into a single deployable device: direct wound coverage via a non-adherent sterile pad, circumferential compression through an elastic wrap, focused force multiplication through a C-shaped Pressure Bar, and tool-free closure via an integrated Closure Bar. This design allows a single operator to apply a complete hemorrhage control dressing without tape, knots, or additional equipment. The non-adherent pad surface prevents adhesion to clotted tissue, protecting wound integrity and reducing re-bleeding risk during removal at higher-echelon care.

The Pressure Bar is the critical differentiator in this device's field performance. After the elastic leader completes one full wrap around the limb, it routes through the Pressure Bar opening and reverses direction, mechanically levering concentrated downward force onto the wound pad. This focused pressure capability addresses bleeds where manual pressure alone is insufficient, making the bandage a viable primary intervention before tourniquet application becomes necessary.

The bandage carries FDA approval as a non-pneumatic tourniquet, but that application is secondary and high-risk, reserved for life-threatening extremity hemorrhage when a dedicated windlass or pneumatic tourniquet is unavailable and direct pressure has failed. The device is not designed for junctional wounds involving the groin, shoulder, or neck, which require junctional tourniquets or hemostatic agents. Readiness depends on packaging integrity: a hard, compressed vacuum seal confirms sterility, while a soft package indicates seal failure and must be removed from operational stock immediately.

Field Application Steps

1. EXPOSURE AND PREPARATION: Cut or remove clothing to fully expose the injury site and identify the primary bleeding point. Pack the wound with gauze if tactically permissible, then tear the outer vacuum-sealed pouch at the pre-cut notches and remove and discard the inner sterile wrapper.

2. WOUND PAD PLACEMENT: Place the white, non-adherent sterile pad directly over the wound source, ensuring full coverage of the bleeding point. Orient the Pressure Bar facing up and away from the skin before proceeding.

3. INITIAL WRAP AND BAR ENGAGEMENT: Hold the pad firmly in place with one hand and wrap the elastic leader once fully around the limb to cover and retain the pad. Route the elastic leader through the opening of the Pressure Bar.

4. APPLY FOCUSED PRESSURE: Pull the elastic leader back over the top of the Pressure Bar in the opposite direction of the wrap, forcing the bar down onto the wound pad. Pull tight until resistance is felt and the bar is firmly seated against the pad surface. This is the most critical step in the procedure.

5. SECURE THE DRESSING: Continue wrapping tightly around the limb, overlapping each previous layer by at least 50 percent and covering all edges of the wound pad. Engage the Closure Bar teeth into the wrap fabric on the side of the limb, not over the Pressure Bar.

6. REASSESSMENT: Check for continued bleeding at the wound edges and assess distal circulation by checking capillary refill. If bleeding persists, apply additional manual pressure over the dressing or apply a proximal tourniquet. Record the time of application and communicate it to receiving personnel at handover to higher-echelon care.
