Quick Overview

A 4x4 sterile gauze pad is a multi-ply cotton wound dressing used for direct pressure hemorrhage control, wound cleaning, and sterile wound coverage by personnel at all training levels from basic first aid providers to advanced clinical operators.

The Luminary 4x4 sterile gauze pad is packaged in a peel-down pouch that maintains sterility against moisture and microbial contamination until the moment of deployment. The pad's multi-ply 100% cotton construction provides rapid absorption of blood and wound exudate, making it adequate for most extremity and torso lacerations. Folded edge construction prevents cotton thread fraying, which directly reduces the risk of foreign body contamination and granuloma formation in the wound bed. The bulk case delivers 1,200 individual pads in 12 dispenser boxes of 100, configured for resupply of medical facilities, emergency vehicles, aid stations, and IFAKs.

This product controls bleeding through mechanical pressure only. It contains no hemostatic agent. For severe arterial or junctional hemorrhage, operators must escalate immediately to a tourniquet or hemostatic dressing. Wound packing and debridement exceed the scope of this product's basic application protocol and require TCCC, TECC, or Paramedic-level certification. Dry gauze must never be applied to significant second- or third-degree burns, as it will adhere to tissue and cause further injury.

Storage readiness requires a cool, dry, climate-controlled environment away from direct sunlight and humidity extremes. Each pouch must be inspected for seal integrity and a current expiration date before deployment. Stock rotation follows First-In, First-Out protocol to ensure field assets are always replenished with viable, sterile product.

Field Application Steps

1. BSI AND SCENE ASSESSMENT: Don nitrile gloves as minimum body substance isolation before contact with the patient. Confirm the scene is secure before approaching the casualty.

2. WOUND EXPOSURE: Expose the injury site by cutting away clothing as necessary. Inspect the wound for embedded objects and identify the primary bleed point before reaching for your dressing.

3. POUCH INSPECTION: Retrieve one or more sterile 4x4 gauze pads from the dispenser box. Inspect each pouch for tears, punctures, or water damage, and confirm the expiration date is current. A compromised pouch equals a non-sterile pad; do not use it.

4. STERILE OPENING: Grasp the pouch at the tear notches and peel it open using both hands. Do not touch the sterile pad surface until you are ready to apply it directly to the wound.

5. DIRECT PRESSURE APPLICATION: Place the center of the pad directly over the bleeding source. Apply firm, continuous pressure using the flat of your hand or fingers and maintain it without interruption for a minimum of 3 to 5 minutes.

6. SATURATED PAD PROTOCOL: If the pad saturates, do not remove it. Place a second sterile pad directly on top of the first and resume firm pressure. Removing the original pad disrupts clot formation and restarts the hemorrhage control timeline.

7. DRESSING SECUREMENT: Once bleeding is controlled, secure the dressing in place using medical tape, an elastic wrap, or a pressure dressing. Ensure the dressing does not restrict circulation.

8. WOUND CLEANING: For wound cleaning after hemorrhage control, moisten a fresh sterile pad with sterile saline or an approved antiseptic. Wipe away from the wound margin to clear debris. Use a new pad for each wipe to prevent recontamination of the wound bed.

9. DISPOSAL AND DOCUMENTATION: Dispose of all used pads as biohazardous waste. Do not reuse, re-sterilize, or repurpose any expended pad. Document the number of pads used and the time pressure was initiated for handoff to the next level of care.
