The ETD consolidates multiple first aid functions into one deployable unit, combining a non-adherent wound pad, elastic compression wrap, and a pressure applicator bar capable of delivering over 30 lbs of focused pressure directly onto the wound bed. The 6-inch width is specifically sized for large surface area wounds, deep lacerations to large muscle groups such as the thigh and buttocks, abdominal eviscerations, and traumatic amputations as a secondary dressing following tourniquet application. This makes it one of the most versatile pressure dressings available for field-ready trauma kits.

The pressure applicator bar is the mechanical core of this system. By feeding the elastic wrap through the bar and reversing direction, the operator levers the bar hard onto the pad, converting wrap tension into direct, concentrated wound pressure. The closure bar at the terminal end of the wrap engages hooks into the underlying layers, securing the dressing without tape, knots, or safety pins, which is a critical capability when operating under stress or with compromised dexterity.

This bandage is a non-invasive, external pressure device and is not a substitute for a CoTCCC-recommended tourniquet in cases of catastrophic arterial extremity hemorrhage. Readiness protocols require pre-deployment inspection of vacuum seal integrity and expiration date. A compromised seal, indicated by a soft or air-filled pouch rather than a hard, brick-like package, requires immediate replacement.

Field Application Steps

1. ESTABLISH SCENE SAFETY AND BSI: Confirm the scene is safe before approaching the casualty. Don nitrile gloves at minimum before making contact with the wound or the patient.

2. EXPOSE AND IDENTIFY THE WOUND: Remove or cut away clothing to fully expose the injury site. Locate the most significant point of bleeding to ensure accurate pad placement.

3. OPEN THE PACKAGE: Tear the vacuum-sealed outer pouch at the pre-cut tear notches without tools. Remove the sterile bandage and keep the non-adherent pad from contacting any non-sterile surface prior to placement.

4. PLACE THE NON-ADHERENT PAD: Place the white, non-adherent pad directly over the wound and the source of bleeding. The non-adherent surface must face the wound to minimize tissue adhesion during removal.

5. EXECUTE THE INITIAL SECURING WRAP: Hold the pad in place with one hand and unroll the elastic leader with the other. Make one complete, snug wrap around the limb over the pad to anchor the dressing and free both hands.

6. POSITION THE PRESSURE APPLICATOR BAR: Continue wrapping until the pressure applicator bar is centered directly over the wound pad. Feed the entire bandage roll through the opening of the pressure bar.

7. APPLY FOCUSED PRESSURE: Forcefully pull the bandage back in the opposite direction to lever the bar hard onto the pad. Visually confirm that significant, focused pressure is being applied to the wound bed.

8. COMPLETE FINAL WRAPS AND SECURE THE DRESSING: Maintain high tension throughout. Continue wrapping with tight, overlapping passes that cover the pressure bar and all edges of the wound pad. Engage the closure bar hooks into the underlying wrap fabric to lock the dressing in place.

9. REASSESS AND DOCUMENT: Check for continued or breakthrough bleeding. If the dressing becomes saturated, apply a second bandage directly over the first or escalate to tourniquet application. Assess distal circulation if conditions permit and note the time of application for handoff to advanced care.
