The kit includes two silicone masks sized for different patient populations, a one-way valve with filter, nitrile gloves, an antiseptic wipe, and a high-visibility red nylon pouch with a clip for rapid deployment. The one-way valve permits airflow from rescuer to patient only, blocking exhaled air, saliva, vomit, blood, and pathogens from reaching the operator. The Adult/Child mask is triangular with an O2 inlet port for supplemental oxygen delivery by qualified EMS personnel. The Infant Mask is small and circular, engineered to seal over both the mouth and nose simultaneously for patients typically under one year old.

Effective ventilation is entirely dependent on achieving and maintaining a proper mask-to-face seal. The E-C Clamp technique is the standard method for holding the mask while maintaining head-tilt and airway position. Each rescue breath should be delivered over approximately one second, with volume calibrated to produce visible chest rise, using cheek puffs as the delivery standard for infants.

All components used during field application are considered single-use and biohazardous after contact with a patient. The one-way valve must never be cleaned or reused. A depleted kit is non-operational and must be formally taken out of service, fully restocked, and documented before returning to readiness.

Field Application Steps

1. CONFIRM SCENE SAFETY AND INITIATE BSI: Verify the scene is safe to approach before contact. Don nitrile gloves immediately upon determining a patient requires intervention as the first step in Body Substance Isolation.

2. ASSESS THE PATIENT: Confirm the patient is unresponsive, apneic, or displaying agonal respirations that require ventilatory support. If a partner is available, direct them to initiate chest compressions while you manage the airway.

3. OPEN THE POUCH AND SELECT THE CORRECT MASK: Unzip the red nylon pouch and lay out all components. Select the Infant Mask (small, circular) for patients typically under one year old. Select the Adult/Child Mask (large, triangular) for all other patients.

4. ASSEMBLE THE ONE-WAY VALVE: Firmly press the one-way valve with filter into the mask port until fully seated. Confirm it is secure before applying the mask to the patient.

5. OPEN THE AIRWAY: For infants, apply a neutral or slightly extended head-tilt to open the airway. Avoid hyperextension in infants, as this can collapse the airway. For adult and child patients, use standard head-tilt chin-lift technique per your CPR protocol.

6. APPLY THE MASK AND ESTABLISH SEAL: Place the mask rim over the patient's mouth and nose, applying gentle, even downward pressure. Use the E-C Clamp technique: three fingers of one hand lift the jaw while the thumb and index finger press the mask to the face to maintain an airtight seal.

7. DELIVER RESCUE VENTILATIONS: Deliver two rescue breaths, each approximately one second in duration, with just enough volume to produce visible chest rise. For infants, use small cheek puffs rather than full lung delivery to avoid barotrauma.

8. MAINTAIN VENTILATION CYCLE: Continue rescue breaths per current CPR guidelines, typically two breaths per 15 or 30 chest compressions depending on the number of rescuers and patient age. Monitor chest rise and fall with each breath. Use the transparent dome as a visual aid to check for lip color changes and airway obstructions, but always confirm ventilation by watching chest movement.

9. CONTINUE UNTIL HANDOFF OR CESSATION CRITERIA ARE MET: Maintain the ventilation cycle until relieved by a higher level of care, the patient resumes effective spontaneous breathing, or the operator is physically unable to continue.

10. EXECUTE POST-INCIDENT PROTOCOL: Remove and properly dispose of gloves. Use the antiseptic wipe for hand hygiene. Dispose of the one-way valve, gloves, and wipe as biohazardous waste. Dispose of the mask per your agency's single-patient-use protocol.

11. RESTOCK AND DOCUMENT: Formally take the kit out of service until fully restocked with a new one-way valve, gloves, and antiseptic wipe. Replace or properly decontaminate the mask per protocol. Document the use and restocking in the appropriate equipment logs before returning the kit to service.
