Quick Overview

An advanced airway management intubation kit is a dedicated, modular storage system that consolidates all endotracheal intubation tools into a single rapidly deployable pouch for use by ALS-credentialed professionals during critical respiratory emergencies.

The kit is built around a Cordura nylon shell with a full-perimeter zipper that opens into a lay-flat clamshell configuration, creating an organized workspace at the patient's side. Rear hook-and-loop strips enable rip-and-run capability, allowing the operator to detach only this module from a primary jump bag without carrying the entire pack to the patient's head. A reflective strip supports rapid identification in low-light environments such as nighttime motor vehicle collisions.

Internal organization is maintained through a tiered elastic loop system sized by tool category. Large loops retain laryngoscope handles, medium loops secure blades including Mac 3, Mac 4, Miller 2, and Miller 3 configurations along with bougies and Magill forceps, and small loops stage ET tubes in sizes 6.0 through 8.0, stylets, 10cc syringes, lubricating jelly packets, and colorimetric capnography detectors. This systematic layout reduces cognitive load during high-stress deployment and keeps advanced airway tools fully segregated from general trauma supplies.

Post-procedure readiness requires immediate biohazard disposal of all single-use items, removal of reusable instruments for sterilization per service guidelines, bag inspection for contamination, and full restock to par level before the unit returns to service. A one-for-one restocking protocol is non-negotiable. The unit is not back in service until every item is verified, expiration dates are confirmed, and all equipment function is checked.

Field Application Steps

1. CONFIRM INTENT: Verbally confirm the decision to intubate with your partner before retrieving any equipment.

2. RETRIEVE AND CARRY THE KIT: Locate the intubation bag. If attached to a primary pack, detach it via the rear hook-and-loop strips and carry it directly to the patient's head.

3. PLACE AND STABILIZE THE BAG: Set the bag on a stable, flat surface, preferably on a sterile drape or the cleanest available area at the scene.

4. OPEN INTO LAY-FLAT CONFIGURATION: Unzip the bag completely around its full perimeter to establish your organized workspace. All equipment is now visible and accessible without digging or searching.

5. SELECT AND VERIFY BLADES AND HANDLE: Based on patient assessment, select a primary and backup blade, for example Mac 4 primary and Mac 3 backup. Attach the primary blade, click it upright, and confirm the light is bright and steady.

6. SELECT ET TUBES: Choose the appropriately sized ET tube for the patient, 7.5mm for an average adult male, and stage a backup one size smaller at 7.0mm.

7. PERFORM CUFF INTEGRITY CHECK: Attach a 10cc syringe to the pilot balloon of the primary ET tube. Inflate the cuff to check for leaks, then deflate it completely before insertion.

8. PREPARE THE STYLET: Insert the stylet into the ET tube, confirming its tip does not extend beyond Murphy's eye. Shape the tube into a hockey stick or C-curve configuration as preferred.

9. CONFIRM ANCILLARIES ARE STAGED: Verify that suction, capnography, and a tube securing device are all immediately available before proceeding with the procedure.

10. EXECUTE CONTAMINATION PROTOCOL POST-PROCEDURE: Dispose of all single-use items including the ET tube, stylet, and syringe per biohazard protocols immediately after the procedure. Do not return any soiled equipment to the bag.

11. REMOVE REUSABLES FOR DECONTAMINATION: Remove the laryngoscope handle and blades from the scene for full decontamination and sterilization per your service's guidelines.

12. RESTOCK TO FULL PAR BEFORE RETURNING TO SERVICE: Inspect the bag for contamination and clean with an approved medical-grade disinfectant if needed. Verify expiration dates on all consumables, confirm every item is at required quantity, and complete a function check on all equipment before the unit is declared back in service.
