The Mayday Bleeding Control Trauma Response Kit is a point-of-injury trauma system that enables non-professional responders to control life-threatening hemorrhage and manage penetrating chest wounds during active threat events, industrial accidents, or mass casualty scenarios before trained EMS arrives.

The kit is organized by operational purpose across five component categories: extremity hemorrhage control, truncal and junctional wound management, penetrating chest injury, responder protection, and ancillary support tools. Core hemorrhage control capability is provided by a one-hand tactical tourniquet, emergency pressure bandages, roll gauze for wound packing, and rapid response wound seals. A Curaplex Select HALO Seal two-pack, including one vented and one non-vented chest seal, addresses penetrating chest trauma and prevents tension pneumothorax. Full PPE including nitrile gloves, safety glasses, a Readi Mask, and a CPR mouthpiece is included to protect the responder during care.

Deployment follows a five-step protocol: confirm scene safety and don PPE, identify life-threatening hemorrhage, execute hemorrhage control using tourniquet or wound packing based on injury location, prevent hypothermia with an emergency survival blanket, and prepare a clear handoff report for arriving EMS. Tourniquet application requires placement 2 to 3 inches above the wound on bare skin, with windlass tightened until bleeding stops and time of application noted. Penetrating chest wounds require the vented HALO seal over the entry wound and the non-vented seal over the exit wound if present.

Readiness protocols require monthly visual inspection of the kit exterior and a full contents inventory every six months or after any deployment. This is a single-use system. Once deployed, the kit must be taken out of service and fully restocked before returning to operational readiness.

Field Application Steps

1. CONFIRM SCENE SAFETY: Verify the immediate area is secure before approaching the casualty. Verbalize "Scene is safe" if operating with other responders.

2. DON PPE: Open the kit and immediately put on nitrile gloves and safety glasses. Apply the Readi Mask if the situation and time permit.

3. EXPOSE THE WOUND: Use the EMT paramedic shears to cut away clothing and fully expose the injury site. Do not attempt to assess or treat through clothing.

4. IDENTIFY HEMORRHAGE TYPE: Look for bright red spurting blood indicating arterial bleeding, or continuous heavy flow soaking through exposed tissue. Determine whether the wound is on a limb, a junctional area, or the chest before selecting your intervention.

5. APPLY TOURNIQUET FOR LIMB WOUNDS: Position the one-hand tactical tourniquet 2 to 3 inches above the wound on bare skin. Tighten the windlass until bleeding stops completely and note the exact time of application.

6. PACK AND DRESS TORSO OR JUNCTIONAL WOUNDS: Pack the wound cavity tightly with roll gauze. Apply firm direct pressure for a minimum of 3 minutes, then secure with the emergency pressure bandage.

7. SEAL PENETRATING CHEST WOUNDS: Wipe the wound area clean. Apply the HALO vented chest seal over the entry wound. If an exit wound is present, apply the non-vented seal over it.

8. PREVENT HYPOTHERMIA: Once all major bleeding is controlled, unfold an emergency survival blanket and cover the casualty from head to toe. Keep the patient calm and still.

9. REASSESS INTERVENTIONS: Continuously monitor the tourniquet to confirm it remains tight and that bleeding has not resumed. Verify all dressings and seals remain in place.

10. PREPARE HANDOFF REPORT: Consolidate the mechanism of injury, all treatments performed with timestamps, and the patient's general condition. Maintain control of the scene and deliver a clear, concise report to arriving EMS or law enforcement.
