Quick Overview

The PerSys Medical Israeli Bandage 6" is an all-in-one emergency pressure dressing used by military personnel, law enforcement, EMS, and trained civilians to control moderate to severe hemorrhage from traumatic wounds when simple direct pressure is insufficient and tourniquet application is not yet indicated.

The Israeli Bandage consolidates four functional components into a single sterile, vacuum-sealed unit: a non-adherent sterile pad sized at 6.5" x 5.25", a 6"-wide by 13.5'-long elastic leader wrap, a rigid plastic Pressure Applicator Bar, and a hooked Closure Bar. The mechanical advantage of this system is its key field capability. When the elastic leader is passed through the Pressure Applicator Bar and reversed in direction, the bar pivots and concentrates up to 30 lbs of focused force directly over the wound site, delivering pressure superior to standard gauze and elastic wrap combinations. No tape, clips, or knots are required for secure closure.

Deployment protocol follows a sequential eight-step procedure applicable to extremity, torso, and junctional wounds. For deep wounds, gauze packing is performed first, with the Israeli Bandage applied over the packing to maintain compression. The first wrap anchors the sterile pad, the second wrap engages the Pressure Applicator Bar, and subsequent overlapping wraps at approximately 50% coverage lock the bar in position before the Closure Bar secures the terminal end. Self-application with one hand is supported by the design, requiring the operator to use the body or available surfaces to maintain tension during initial wrapping.

This device is a bridging instrument for hemorrhage control, not a definitive treatment. Readiness protocol requires monthly inspection of the vacuum seal, packaging integrity, and expiration date. A soft or puffy package indicates a compromised seal and must be relegated to training use only. The bandage is single-use and must be replaced immediately after any operational deployment.

Field Application Steps

1. DON PERSONAL PROTECTION: Put on BSI gloves before making contact with the casualty. If gloves are unavailable, proceed and address exposure risk at the earliest opportunity.

2. EXPOSE AND ASSESS THE WOUND: Cut or remove clothing to gain an unobstructed view of the injury site. Visually identify the primary source of bleeding before opening the bandage.

3. PACK DEEP WOUNDS FIRST: If the wound is deep and you are trained in wound packing, pack the cavity with gauze before applying the bandage. The Israeli Bandage will be applied over this packing to hold it in place and maintain compression.

4. OPEN PACKAGING: Tear the outer and inner vacuum-sealed packaging at the designated tear notches. This process is designed for rapid execution with gloved hands.

5. PLACE THE STERILE PAD ON THE WOUND: Position the non-adherent sterile pad directly over the injury, ensuring full coverage of the wound site. Do not allow the pad surface to contact non-sterile areas before placement.

6. EXECUTE THE FIRST WRAP: Wrap the elastic leader once completely around the limb, passing over the pad to anchor it firmly against the wound.

7. ENGAGE THE PRESSURE APPLICATOR BAR: On the second wrap, pass the elastic leader through the Pressure Applicator Bar, with the bar positioned directly over the wound pad.

8. APPLY MECHANICAL PRESSURE: Pull the elastic leader in the opposite direction from your initial wrap. The bar will lever downward, concentrating up to 30 lbs of focused force onto the wound pad.

9. CONTINUE WRAPPING TO LOCK THE BAR: Wrap the elastic leader tightly up the limb, overlapping each layer by approximately 50%. This progression locks the Pressure Applicator Bar in its compressed position.

10. SECURE THE CLOSURE BAR: Slide the hooked terminal end of the elastic leader under the last two bandage layers to lock the dressing. No tape, clips, or knots are required.

11. ASSESS HEMORRHAGE CONTROL: Visually check for continued bleeding. If bleeding is not controlled, apply a second dressing over the first. If hemorrhage continues and the wound location permits, escalate to tourniquet application.

12. MONITOR CIRCULATION ON EXTREMITY WOUNDS: If applied to a limb and you are trained to do so, assess for distal pulses after application. Do not remove the bandage in the field to inspect the wound. Wound inspection is reserved for advanced medical providers.
