Quick Overview

The QuikClot EMS 4" x 4" Hemostatic Dressing is a kaolin-impregnated gauze pad used by first responders, law enforcement, EMS personnel, and prepared civilians to achieve temporary external control of moderate to severe traumatic bleeding that standard gauze cannot manage.

The dressing's active ingredient, kaolin, is an inert inorganic mineral impregnated directly into the gauze fibers. Upon contact with blood, kaolin activates Factor XII and accelerates the body's natural clotting cascade. The mechanism is nonexothermic, meaning it generates no heat and poses zero risk of thermal burns to the patient or operator. This makes it a reliable hemostatic capability in both controlled and austere field environments.

The 4-ply non-woven gauze construction provides absorption and focused pressure across common laceration sizes, while the pliable nature of the pad allows conformation to uneven body surfaces and packing into wound cavities. The dressing is designed for deployment in junctional bleeding zones where a tourniquet cannot be applied, including the neck, groin, and scalp, as well as extremity wounds with venous or arterial bleeding that does not meet tourniquet criteria. It mirrors standard gauze application protocol, reducing the cognitive load on operators working under stress.

Readiness sustainment for this device requires routine inspection of the ruggedized foil pouch for packaging integrity, verification of the printed expiration date, and proper storage in a cool, dry location away from extreme temperature fluctuations. Any dressing that is used, expired, or has a compromised package must be removed from service and replaced immediately to maintain mission readiness.

Field Application Steps

1. SCENE SAFETY AND BSI: Confirm the scene is safe before approaching the patient. Don non-latex gloves at minimum to establish Body Substance Isolation and protect against bloodborne pathogen exposure.

2. EXPOSE AND IDENTIFY THE WOUND: Remove clothing and debris to fully expose the injury site. Wipe away pooled blood where possible to identify the most active bleeding point before applying the dressing.

3. OPEN PACKAGING: Grasp the foil pouch and tear at the designated tear notches on the edges. Remove the sterile QuikClot EMS dressing and avoid contact with any non-sterile surface before application.

4. APPLY DRESSING DIRECTLY TO THE BLEEDING SOURCE: Place the QuikClot gauze pad directly onto the most active bleeding source, ensuring maximum surface contact with the wound bed. For puncture wounds, push a corner of the pad into the wound cavity to establish contact with the bleeding point.

5. APPLY FIRM, CONTINUOUS DIRECT PRESSURE: Place your hand or hands directly over the pad and apply firm, unyielding pressure for a minimum of 3 full minutes. Use body weight if necessary, and do not lift or release pressure to check the wound, as this disrupts clot formation.

6. ASSESS FOR BLEEDING CONTROL: After 3 minutes, slowly release pressure while leaving the dressing in place. Inspect the edges of the dressing for continued blood flow. If bleeding is controlled, proceed to secure. If bleeding continues, apply a second dressing on top of the first and reapply pressure for 3 to 5 additional minutes.

7. SECURE THE DRESSING: Leave the QuikClot dressing in place and secure it with a pressure dressing or elastic conforming bandage. Wrap firmly enough to maintain pressure without cutting off distal circulation. Verify a distal pulse and check capillary refill after securing.

8. MONITOR AND PREPARE FOR TRANSPORT: Keep the patient calm and warm. Continuously monitor the dressing for signs of re-bleeding. Prepare the patient for immediate transport to a medical facility for definitive care.
