Quick Overview

The R&B Bariatric Transfer Sheet (Model 181YL) is a professional-grade soft litter device used by emergency medical operators to execute planned, short-distance lifts and transfers of bariatric patients weighing up to 700 lbs without rigid equipment.

The device is constructed from two layers of 14 oz. nylon-reinforced vinyl measuring 60 by 80 inches, with a structural webbing matrix sewn through the center to distribute patient weight evenly and prevent sheet collapse under load. Ten webbing handholds, five per long side, allow operator scaling from a minimum of four to a maximum of ten personnel depending on patient weight and environmental conditions. All handhold attachment points use Box-X stitching rated to the full 700 lb. static load capacity. The high-visibility yellow construction supports situational awareness in low-light deployment environments.

This transfer sheet is designed for four primary use cases: patient transfer from floor, bed, or chair to a bariatric stretcher; lift assist for patients unable to rise independently; confined space extraction where rigid stretchers cannot be maneuvered; and biohazard isolation, where the fluid-impervious vinyl surface prevents cross-contamination between patient and surrounding surfaces. The sheet is explicitly not a spinal immobilization device and provides no rigidity for cervical spine precautions.

Post-deployment protocol requires gross decontamination on-scene if heavily soiled, full decontamination at the station using approved non-bleach medical-grade disinfectant, complete air drying before storage, and a detailed inspection of webbing, stitching, and vinyl surface before the device is returned to mission-ready status.

Field Application Steps

1. CONFIRM NEED AND ASSIGN ROLES: The Team Leader verifies that patient weight and condition require the Bariatric Transfer Sheet, then assigns a minimum of 6 to 8 operators to handhold positions scaled to patient weight and the complexity of the environment.

2. CLEAR THE EGRESS PATH: Remove all furniture, obstacles, and hazards between the patient's location and the staged bariatric stretcher before device deployment begins.

3. BRIEF THE PATIENT: If the patient is conscious and alert, explain the full procedure clearly to gain cooperation and reduce anxiety before any movement occurs.

4. UNPACK AND INSPECT: Remove the sheet from its storage bag and visually inspect all handholds, webbing, and vinyl surfaces for cuts, fraying, punctures, or damaged stitching before placing it near the patient.

5. POSITION THE SHEET: Unfold the sheet and lay it flat alongside the patient on the ground with the webbing matrix centered relative to the patient's anticipated body position.

6. LOG-ROLL AND PLACE: With the Team Leader controlling the head and cervical spine if protocol indicates, log-roll the patient away from the sheet, slide the sheet underneath, then gently roll the patient back onto the sheet.

7. CENTER THE PATIENT: Confirm the patient's center of mass is directly over the structural webbing matrix and positioned evenly between both rows of handholds. Ensure all extremities are within the perimeter of the sheet.

8. POSITION OPERATORS AND ESTABLISH GRIP: Operators take assigned handhold positions distributed evenly on both sides. Each operator secures a firm two-handed grip on their webbing handhold, feet shoulder-width apart, knees bent, back straight.

9. EXECUTE THE LIFT: The Team Leader calls "All positions ready?" and waits for verbal confirmation from every operator before issuing "Prepare to lift on my command... LIFT!" All operators lift simultaneously and smoothly using leg and core muscles, raising the patient only high enough to clear the ground and immediate obstacles.

10. MANEUVER AND TRANSFER: Move slowly and deliberately toward the bariatric stretcher. The Team Leader calls out directions and warns of upcoming obstacles. Halt all movement once the patient is positioned directly over the target surface.

11. LOWER AND REMOVE: The Team Leader issues "Prepare to lower on my command... LOWER!" All operators lower in a controlled, simultaneous manner until the patient rests fully on the stretcher. Once the patient is secure, slide the sheet out from underneath.

12. DECONTAMINATE, INSPECT, AND STOW: Perform gross decontamination on-scene if heavily soiled. At the station, clean all surfaces including webbing with approved non-bleach medical-grade disinfectant, air dry completely, conduct a post-mission inspection of webbing, stitching, and vinyl, and return the sheet to its storage bag.
