The Rothco 16-Piece Surgical Instrument Kit is a field-deployable instrument set used by trained personnel for wound inspection, debridement, foreign body removal, and laceration closure in austere or remote environments where definitive medical care is unavailable or significantly delayed.

The kit is organized across four operational categories: wound closure and management, incision and exploration, illumination and site prep, and a MOLLE-compatible carrier system. Core instruments include a needle holder, three hemostats, surgical scissors, a scalpel handle with two blades, a needle probe, tweezers, a suture set, and a pen light. All components are supplied non-sterile, and field sterilization via boiling stainless steel instruments in clean water for a minimum of 10 minutes is a critical protocol requirement before any instrument contacts tissue.

This kit is a supplemental instrument set, not a comprehensive trauma or first aid system. It contains no tourniquets, hemostatic agents, gloves, bandages, or irrigation solution. Deployment is authorized only after all life-threatening hemorrhage has been controlled by other means. Operators are responsible for augmenting the kit with sterile gloves, irrigation solution, pressure dressings, and a sharps disposal container to meet full readiness standards.

Readiness sustainment requires inventory and functional checks every 90 days, replacement of all single-use consumables after each deployment, and re-sterilization of all reusable instruments before the kit is returned to service. The kit is explicitly restricted to personnel with documented medical training operating within their established scope of practice.

Field Application Steps

1. SAFETY AND ASSESSMENT: Confirm scene security and verify that all life-threatening hemorrhage has been controlled before deploying this kit. Don PPE including sterile gloves and eye protection, neither of which is included in the kit.

2. EQUIPMENT STAGING: Open the MOLLE pouch and lay all instruments on the cleanest available surface, such as the inside of a sterile dressing package or a sterile drape. Arrange components for sequential access before beginning the procedure.

3. WOUND ILLUMINATION AND INSPECTION: Use the pen light to examine the wound cavity for deep-tissue damage, tendon or nerve involvement, and retained foreign bodies. Assess the viability of tissue at the wound edges before proceeding.

4. FIELD STERILIZATION OF INSTRUMENTS: Boil all stainless steel instruments that will contact tissue in clean water for a minimum of 10 minutes. Chemical sterilization is an acceptable alternative when appropriate agents are available. The included alcohol and BZK wipes are insufficient for instrument sterilization and must not be used as a substitute.

5. WOUND IRRIGATION AND PREP: Irrigate the wound vigorously with a minimum of 0.5 to 1 liter of potable or sterile solution, which is not included in the kit. Clean the intact skin surrounding the wound perimeter with BZK antiseptic wipes, wiping away from the wound edge. Do not introduce alcohol or BZK wipes into the wound cavity, as both agents are cytotoxic to tissue.

6. DEBRIDEMENT: Using hemostats or tweezers, remove visible debris and foreign bodies from the wound. If non-viable tissue is present, debride using the scalpel and surgical scissors as indicated by training and scope of practice.

7. WOUND CLOSURE: Load the suture needle into the needle holder and close the laceration using the appropriate technique for the wound presentation, such as simple interrupted sutures. Cut suture material with the surgical scissors after each knot is secured.

8. POST-PROCEDURE SUSTAINMENT: Apply a sterile dressing, not included, and monitor the patient for signs of infection or deterioration. Dispose of all single-use sharps in a puncture-proof biohazard container. Clean all reusable instruments with soap and water, dry completely to prevent corrosion, re-sterilize, and return them to the pouch. Replace all consumed single-use items before the kit is considered mission-ready.
