Quick Overview

The Rothco EMS Medical Field Pouch is a compact, rapidly deployable trauma carrier used by first responders, law enforcement officers, and tactical operators to manage immediate life threats including major hemorrhage, airway obstruction, and penetrating chest trauma at the point of injury.

The pouch is constructed from 1000 Denier polyester and measures 7 inches by 9 inches by 4 inches, providing a rigid chassis that maintains shape and enables consistent access under field conditions. Its quick-release buckle is designed for one-handed operation and outperforms zippers or hook-and-loop closures that can fail when contaminated with mud, snow, or blood. A universally recognized Star of Life emblem on the exterior flap provides immediate visual identification, preventing confusion with tactical or utility pouches during high-stress deployment.

Three expandable zippered compartments support a MARCH protocol loadout: the top compartment holds hemorrhage control supplies including tourniquets, pressure dressings, and hemostatic gauze; the middle compartment holds airway and breathing management tools including chest seals and nasopharyngeal airways; and the bottom compartment holds assessment tools and secondary supplies. An open-top pocket under the main flap allows retrieval of nitrile gloves and triage cards without operating a zipper, preserving speed of access during initial patient contact.

This pouch is designed to augment a comprehensive medical bag, not replace it. It is sized and organized for single-patient stabilization, delivering critical capability directly at the point of injury until a higher level of care can be established. Operators must be trained and authorized to use every item they load into the carrier.

Field Application Steps

1. PERFORM PRE-DEPLOYMENT INSPECTION: Before each shift or weekly at minimum, actuate the quick-release buckle to confirm it locks and releases correctly, cycle all three zippers fully to check for binding or torn tracks, and inspect the pouch body, seams, and handles for tears or fraying. Verify all contents are present, sealed, and within expiration dates using a laminated loadout list kept inside the pouch.

2. CONFIGURE COMPARTMENTS TO MARCH PROTOCOL: Load the top compartment with hemorrhage control supplies: tourniquets, pressure dressings, and hemostatic gauze. Load the middle compartment with airway and breathing supplies including nasopharyngeal airways and chest seals, and load the bottom compartment with assessment tools, nitrile gloves, trauma shears, and minor wound care items. Stage nitrile gloves, a triage card, and a permanent marker in the open-top pocket for access without unzipping.

3. STAGE THE POUCH FOR DEPLOYMENT: For shoulder carry, attach and adjust the removable shoulder strap for cross-body or single-shoulder configuration. For vehicle staging or grab-and-go deployment, position the pouch on a shelf or in a bag with the top carry handle accessible for immediate retrieval.

4. EXECUTE INITIAL APPROACH AND STAGING: On arrival at the casualty, drop to a knee at the patient's head. Rotate the pouch to your front or place it beside the casualty's head on a clean, stable surface, positioning upwind if chemical agents are present.

5. ACCESS THE POUCH: Actuate the quick-release buckle with one hand and sweep the flap fully open to expose all three zippered compartments and the open-top pocket simultaneously.

6. RETRIEVE TOOLS BY COMPARTMENT PRIORITY: Retrieve nitrile gloves from the open-top pocket without operating a zipper. For hemorrhage, open the top compartment first. For airway or chest trauma, access the middle compartment. Base compartment selection on your visual assessment of the casualty's immediate life threats.

7. MANAGE THE POUCH DURING INTERVENTION: If movement is anticipated after an intervention is complete, close and secure the buckle to prevent spill-out. In a static scene, compartments may remain open for continued access without re-securing between interventions.

8. TRANSITION TO HIGHER CARE: Manage immediate life threats until relieved by EMS personnel. Report every item used from the pouch to the incoming crew to ensure continuity of care and accurate resupply.

9. DECONTAMINATE AFTER USE: Remove all contents and empty every compartment completely. Clean all surfaces with an approved intermediate-level disinfectant, paying specific attention to areas exposed to blood or bodily fluids. Allow the pouch to air dry completely. Do not machine wash.

10. RESTOCK AND DOCUMENT: Replace all used or compromised items and return the pouch to its standard loadout configuration using the laminated loadout list. Log the use of the kit and its resupply per your organization's policies to support supply tracking and replenishment justification.
