Quick Overview

The Rothco Fast Action First Aid Tourniquet Pouch is a MOLLE-compatible IFAK carrier designed for tactical, patrol, and high-risk field operations, enabling operators to access hemorrhage control and trauma supplies with a single gross motor movement under extreme stress.

The pouch addresses two critical failure points in point-of-injury care: the delay caused by a tourniquet buried inside a main compartment, and the inability to operate standard zippers when fine motor skills degrade under adrenaline load. The exterior tourniquet pocket provides immediate one-handed access to a staged TQ without opening the main body. The fast action pull tab deploys the entire main compartment in a single downward rip, actuating the U-shaped double zipper and dropping the front flap fully open. This design aligns directly with the M-A-R-C-H algorithm, supporting massive hemorrhage control, airway and respiration access, and circulation interventions in sequence.

The pouch is constructed from 600 denier polyester with metal drainage grommets at the base, providing water resistance and fluid evacuation in field conditions. Interior elastic pockets organize flat-packed supplies including hemostatic gauze, compressed gauze, and vented chest seals. A dedicated internal shear retention loop secures trauma shears tip-down, keeping them isolated from the zipper path. The MOLLE/PALS webbing mounts to plate carriers, battle belts, or vehicle panels in the standard vertical orientation with the pull tab at the top.

Over-packing is the primary failure point for this system. Excessive contents create zipper tension that prevents the rip-open function from actuating correctly. Operators must verify the main compartment closes and opens without significant resistance during every pre-mission inspection.

Field Application Steps

1. STAGE THE TOURNIQUET: Fold a TCCC-approved tourniquet such as a CAT Gen 7 or SOFTT-W into deployment configuration with the windlass clip secured and the time strap prepped. Insert it into the exterior tourniquet pocket with the pull tab oriented for intuitive draw, then secure the hook-and-loop retention flap.

2. LOAD FLAT ITEMS INTO INTERIOR ELASTIC POCKETS: Place hemostatic gauze and compressed gauze together in one interior elastic pocket and vented chest seals in the other. Contents must sit snug but remain individually retrievable without digging. Do not overstuff.

3. SEAT TRAUMA SHEARS IN THE RETENTION LOOP: Insert trauma shears tip-down into the internal shear retention loop until fully seated. Confirm the shears do not obstruct the zipper path before proceeding.

4. CONDUCT FINAL LOADOUT CHECK: Close both main compartment zippers completely and confirm the fast action pull tab is exposed and accessible, not tucked or pinned. If the zipper resists closing, remove contents until the pouch closes without tension. A difficult close means the rip-open function will likely fail under duress.

5. MOUNT TO PLATFORM: Attach the pouch vertically to your platform using MOLLE straps, positioning it at the 5 or 7 o'clock position on a duty belt or on the front or side cummerbund of a plate carrier. Confirm the location is accessible by both hands.

6. VERIFY ORIENTATION: Ensure the fast action pull tab is positioned at the top of the pouch and the exterior tourniquet pocket faces outward away from the body.

7. CONDUCT PRACTICE DRAWS: In a safe training environment with an unloaded weapon, rehearse draws with both dominant and non-dominant hands. Confirm the mounting position does not interfere with weapon manipulation, movement, or vehicle operation.

8. EXECUTE TOURNIQUET DEPLOYMENT AT POINT OF INJURY: For massive extremity hemorrhage, rip open the hook-and-loop flap on the exterior tourniquet pocket with either hand, retrieve the tourniquet, and apply it high and tight to the injured limb. This is always the first action and must be performed without delay.

9. DEPLOY MAIN COMPARTMENT UNDER DURESS: To access wound packing supplies, chest seals, shears, or a second tourniquet, grasp the fast action pull tab firmly and pull straight down toward the ground in one continuous forceful motion. This actuates the U-shaped double zipper and forces the front flap fully open.

10. RETRIEVE AND TREAT: With the pouch splayed open, identify the required item from its designated pocket or retention loop and proceed with self-aid or buddy-aid consistent with your certified level of training.
