Quick Overview

The Rothco MOLLE Tactical First Aid Kit is a Basic Trauma and First Aid Kit designed for individual carry on MOLLE/PALS platforms, enabling operators to provide immediate point-of-injury self-aid or buddy-aid for minor to moderate traumatic injuries until advanced medical care is available.

This kit's operational capability covers moderate hemorrhage control through direct pressure and pressure bandaging, wound cleaning and dressing for superficial lacerations, burns, and abrasions, minor musculoskeletal injury management using cold therapy, and removal of small foreign bodies. Its 25-plus components are grouped by operational purpose, including military bandages, sterile pads, a Telfa non-adherent pad for burn management, antiseptic solutions, and an instant cold pack for sprain protocol. The MOLLE pouch features internal elastic loops for rapid no-look access and attaches securely to load-bearing equipment via integrated MOLLE straps.

Operators must understand the critical capability gap this kit carries into the field. It does not include a CoTCCC-recommended tourniquet, hemostatic gauze, occlusive chest seals, or a nasopharyngeal airway. By modern tactical medical standards, it is not a comprehensive IFAK and is inadequate for managing life-threatening arterial hemorrhage. It is designated for environments where the risk of massive hemorrhage or penetrating torso trauma is low, or as a supplement to a more advanced trauma kit.

Readiness sustainment requires inspection every 30 days and after each use, covering pouch integrity, item expiration on antibiotic ointment, prep pads, and aspirin, and package seal integrity on all sterile components. Per Luminary's replenishment doctrine, a partially stocked kit is a failed kit, and any deployed component must be replaced before the next operational period.

Field Application Steps

1. CONFIRM SCENE SAFETY: Verify the immediate area is clear of the initial hazard and any secondary threats before approaching the casualty. Do not initiate treatment in an unsecured environment.

2. DON EXAMINATION GLOVES: Open the kit and put on the single included pair of examination gloves before making any patient contact. This is a non-negotiable first action under BSI protocol.

3. EXPOSE THE INJURY: Move clothing away from the wound site to fully visualize the extent of the laceration and identify any additional injuries before initiating hemorrhage control.

4. APPLY DIRECT PRESSURE: Open one 3"x3" sterile pad and place it directly onto the laceration. Apply firm, sustained direct pressure with your gloved hand for a minimum of 1 to 2 minutes.

5. ASSESS BLEEDING AND ADD PADS IF NEEDED: After 1 to 2 minutes, assess whether bleeding is controlled. If the pad is saturated and bleeding continues, add a second sterile pad on top. Do not remove the original pad.

6. APPLY MILITARY BANDAGE AS PRESSURE DRESSING: Place the pad of one military bandage over the sterile pads on the wound. Wrap the elastic leader tightly around the limb, snug enough to maintain pressure without cutting off circulation, and secure with the integrated clips.

7. CHECK DISTAL CIRCULATION: Verify a radial pulse or capillary refill in the fingers of the injured limb to confirm the pressure dressing is not functioning as an unintentional tourniquet.

8. CLEAN AND DRESS SECONDARY ABRASIONS: Use a BZK antiseptic towelette or hydrogen peroxide to clean minor abrasions. Apply a thin layer of antibiotic ointment and cover with an appropriately sized adhesive bandage.

9. MONITOR FOR SHOCK: Watch the casualty for signs of shock, including pale skin, confusion, and rapid pulse. Keep the casualty calm, still, and warm while awaiting evacuation.

10. SECURE THE KIT AND REPORT: Repack unused items, close the pouch, and report the injury to team leadership. Evacuate the casualty to a higher level of medical care for evaluation and possible wound closure.
