The SOF Tactical Tourniquet (SOF-TT) is a professional-grade mechanical compression device used by military operators, rescue personnel, and trained civilians to achieve immediate arterial occlusion and control life-threatening extremity hemorrhage when direct pressure is ineffective.

The SOF-TT is specifically engineered for deployment in adverse field conditions where dirt, mud, snow, ice, or water saturation would compromise hook-and-loop based systems. Its 1.5-inch compression band distributes circumferential force over a larger surface area to increase occlusion effectiveness while reducing localized tissue damage. The windlass handle is machined from aircraft-grade aluminum, providing the torque capacity required to occlude the femoral artery in lower-extremity hemorrhage scenarios without risk of material failure under extreme temperatures or high mechanical load.

The occlusion system combines three redundant securing components: the aluminum windlass, dual high-impact acetyl locking rings, and a safety set screw that prevents windlass slippage during patient movement or evacuation. Bleeding cessation is the primary endpoint of application, confirmed by absence of a distal pulse. Significant patient pain during application is a clinical indicator of effective arterial occlusion and is not a reason to loosen the device.

Readiness protocol requires the SOF-TT to be staged with the running end pre-fed through the buckle, excess strap secured, and the windlass lightly engaged in a tri-ring for immediate one-handed deployment. Pre-duty inspection covers the strap, buckle, rings, windlass, and set screw. Any device that fails inspection must be removed from service immediately and replaced before the operator returns to a field-ready status.

Field Application Steps

1. SITE SELECTION: Position the tourniquet band high on the limb, between the wound and the torso. Do not place the device over a joint, a holster, or any bulky pocket item.

2. INITIAL SECURING: Feed the running end of the compression band through the quick-connect buckle and pull with maximal force to remove all slack. A correctly tightened strap will not allow more than two fingers between the band and the limb.

3. APPLY MECHANICAL TENSION: Grasp the aluminum windlass handle and turn it to tighten the compression band. Continue turning until all bright red arterial bleeding has ceased. Bleeding control is the primary endpoint, not a specific number of turns.

4. CONFIRM OCCLUSION: Check for absence of a distal pulse, radial for arm application or pedal for leg application, to confirm full arterial occlusion. Significant patient pain is an indicator of effective application. Do not loosen the device in response to patient complaint.

5. SECURE WINDLASS, PRIMARY LOCK: While maintaining tension on the windlass, lock the handle into one of the two Dual Locking Rings. Ensure the windlass is fully seated and secure before proceeding.

6. ENGAGE REDUNDANT LOCK: Tighten the Safety Set Screw finger-tight to prevent the windlass from being inadvertently dislodged during patient movement or casualty evacuation.

7. DOCUMENT TIME OF APPLICATION: Using a permanent marker, write the time of tourniquet application on the white time strap on the device or directly on the casualty's forehead, for example T: 1430. This information is critical for receiving medical personnel.

8. SECOND TOURNIQUET PROTOCOL: If bleeding persists after full windlass tension on a large muscle mass such as a well-developed thigh, apply a second SOF-TT immediately proximal to the first. Do not loosen or remove the first device.
