Field Application Steps

1. ESTABLISH SCENE SAFETY AND ASSESS THE CASUALTY: Before initiating any intervention, confirm the environment is safe to operate in. Identify the injury pattern: catastrophic limb hemorrhage or penetrating chest wound, as each requires a different protocol.

2. DON PROTECTIVE GLOVES: Put on the nitrile gloves from the kit before making contact with the casualty. This protects both the operator and the casualty from cross-contamination.

3. IDENTIFY THE BLEED SOURCE: Expose the wound site by cutting away clothing if necessary. Determine whether the hemorrhage is from a limb and cannot be controlled by direct pressure alone, which requires tourniquet application and wound packing.

4. APPLY THE TOURNIQUET FOR LIMB HEMORRHAGE: Position the tourniquet two to three inches above the wound on the limb, not over a joint. Tighten until bleeding stops, note the time of application on the tourniquet or casualty, and do not remove it.

5. PACK THE WOUND IF TOURNIQUET PLACEMENT IS NOT POSSIBLE: For wounds in junctional areas where a tourniquet cannot be seated, pack the wound firmly with the hemostatic gauze, applying direct pressure for a minimum of three minutes. Maintain pressure continuously.

6. SECURE THE WOUND PACKING WITH A PRESSURE BANDAGE: Apply the pressure bandage over the packed wound to maintain compression. Ensure the bandage is tight enough to sustain pressure but does not cut off circulation to uninjured tissue.

7. ADDRESS A PENETRATING CHEST WOUND WITH A CHEST SEAL: For open wounds to the torso within the area between the neck and the navel, wipe the skin dry and apply the vented chest seal directly over the wound. Ensure full adhesion around the wound margins to prevent air from entering the chest cavity.

8. MONITOR THE CASUALTY AND INITIATE EVACUATION: Reassess the casualty continuously for changes in responsiveness, breathing, and bleeding status. Initiate immediate evacuation to an advanced medical facility without exception, as this kit provides temporary life-sustaining measures only.
