Quick Overview

Transcend Glucose Gel is a 15-gram oral dextrose intervention used by EMS personnel, diabetic individuals, caregivers, and athletic support staff to rapidly reverse symptomatic hypoglycemia in conscious patients who can manage their own airway.

The gel delivers 15 grams of pure glucose directly through the oral mucosa and upper GI tract, requiring minimal digestion for rapid blood glucose elevation. This aligns with the American Diabetes Association's 15-15 Rule: administer 15 grams of fast-acting glucose, wait 15 minutes, and reassess. The gel consistency is a deliberate field-ready advantage over thin liquids, reducing aspiration risk, and over solid foods, reducing choking hazard. Each single-dose pouch is sealed in a multi-layer foil and polymer construction that resists moisture, UV degradation, and pressure damage within medical kits and load-bearing equipment.

Deployment of this intervention requires confirming that the patient is conscious, has a patent airway, and demonstrates an intact swallow reflex before administration. The gel is placed into the buccal pouch between the cheek and gum, then dispensed slowly from bottom to top of the pouch. Forcing gel directly to the back of the throat is a protocol violation that increases aspiration risk. If blood glucose remains below 70 mg/dL after 15 minutes, a second 15-gram pouch is indicated, followed by transport per local protocol.

This product is indicated exclusively for hypoglycemia and must never be administered to an unconscious patient or one presenting with hyperglycemia. It is an oral-only intervention and does not replace advanced life support measures including IV dextrose or glucagon for severe or unresponsive cases. Readiness requires periodic inspection of seal integrity, expiration date verification, and immediate replacement of any pouch used in operational or training scenarios.

Field Application Steps

1. CONFIRM PATIENT INDICATIONS: Verify the patient is conscious, has a patent airway, and possesses an intact gag reflex. Ask directly, "Can you swallow for me?" to confirm readiness for oral administration.

2. VERIFY HYPOGLYCEMIA: Confirm low blood glucose via fingerstick glucometer if available, with a threshold below 70 mg/dL as the clinical indicator. If a glucometer is not available, proceed based on strong clinical signs including diaphoresis, tremors, and altered mentation in a patient with known diabetes history.

3. INSPECT THE POUCH: Remove one Transcend Glucose Gel pouch from your kit and visually confirm the seal is intact with no evidence of puncture, leaking, or swelling. Do not deploy a compromised pouch.

4. OPEN THE POUCH: Locate the pre-scored tear notch on the upper edge of the pouch. Grasp firmly on both sides of the notch and tear open in a single, deliberate motion requiring no tools.

5. POSITION THE PATIENT: Ensure the patient is seated upright or semi-upright before administration. This positioning is non-negotiable and directly reduces aspiration risk during gel delivery.

6. ADMINISTER THE GEL: Place the open end of the pouch into the patient's buccal pouch, the space between the cheek and gum. Squeeze slowly from the bottom of the pouch upward, dispensing the entire 15-gram contents. Do not squirt gel directly to the back of the throat.

7. INSTRUCT THE PATIENT TO SWALLOW: Direct the patient to swallow the gel after it has been deposited into the buccal space. Do not force swallowing or rush the process.

8. INITIATE THE 15-MINUTE REASSESSMENT WINDOW: Note the exact time of administration and monitor the patient's airway and level of consciousness continuously throughout the wait period.

9. REASSESS BLOOD GLUCOSE AT 15 MINUTES: Perform a follow-up fingerstick at the 15-minute mark. If symptoms persist or blood glucose remains below 70 mg/dL, administer a second 15-gram pouch and initiate transport per local protocol.

10. PROVIDE SUSTAINED FUEL ONCE STABILIZED: After the patient's blood glucose stabilizes, provide a complex carbohydrate and protein source such as crackers or a sandwich. This step prevents rebound hypoglycemia after the initial glucose intervention.
