The Trauma2 is built around a six-component system grouped by function: pressure and constriction, mechanical advantage and locking, and securing with documentation. The constricting band applies circumferential force to the limb, the pass-through buckle removes all slack before windlass engagement, and the windlass rod acts as a lever to generate the extreme mechanical force required for arterial occlusion. The windlass clip locks the rod in place, and the securing strap provides a secondary retention layer with a dedicated time stamp field for recording application time, critical information for receiving surgical teams.

Effective deployment requires the tourniquet be positioned 2 to 3 inches proximal to the wound, never over a joint or bulky material. The band must be cinched to maximum tightness before the windlass is turned. Windlass rotation continues until hemorrhage stops completely and no distal pulse is detectable at the wrist or ankle. If bleeding persists after full tightening, protocol calls for a second tourniquet applied directly proximal to the first.

Readiness discipline is as important as application technique. The device should be staged outside of original packaging with the strap pre-routed through the buckle and secured with a breakable rubber band for one-hand access. Periodic inspection for band fraying, polymer cracking, and hook-and-loop degradation is required. A permanent marker must be staged with the tourniquet at all times. Once applied, the device is single-use and must only be removed by qualified medical personnel in a clinical setting.

Field Application Steps

1. CONFIRM INDICATION AND EXPOSE THE LIMB: Identify life-threatening extremity hemorrhage that cannot be controlled by direct pressure. Cut or tear away clothing to fully expose the affected arm or leg without wasting time locating the exact wound source.

2. POSITION THE TOURNIQUET: Route the constricting band around the limb and place it 2 to 3 inches proximal to the wound. If the wound is near a joint, position the tourniquet above the joint. Never apply directly over a joint or over bulky items in a pocket.

3. ROUTE AND CINCH THE BAND: Feed the band tip through the pass-through buckle and pull with maximum force to remove all slack from the system. The tourniquet must be extremely tight before the windlass rod is ever turned. Secure the band back onto itself using the hook-and-loop surface.

4. ENGAGE THE WINDLASS ROD: Twist the windlass rod until hemorrhage stops completely. Continue turning until you can no longer feel a distal pulse at the wrist or ankle on the affected limb.

5. LOCK THE WINDLASS CLIP: Seat the windlass rod securely into one side of the windlass clip. Confirm it is fully seated and will not slip out during casualty movement.

6. SECURE THE STRAP AND DOCUMENT APPLICATION TIME: Route the white securing strap over the windlass clip, through the opposite side, and secure it back onto itself. Immediately record the exact time of application on the time stamp field using a permanent marker, for example T=1430.

7. REASSESS FOR HEMORRHAGE CONTROL: Continuously reassess the wound site for continued bleeding. If bleeding persists, confirm the tourniquet is at maximum tightness. If hemorrhage continues, apply a second tourniquet directly proximal to the first and communicate tourniquet application to all responding personnel.

8. ONE-HANDED SELF-AID VARIATION: If self-applying with a single functional hand, pre-form the band into a loop large enough to slide over the hand or foot before incident or at the point of injury. Slide the loop up the injured limb to 2 to 3 inches above the wound, pull the strap end away from your body to remove all slack, then follow steps 4 through 7 using your uninjured hand.
