The kit provides a layered hemorrhage control capability built around hemostatic BleedStop bandages, abdominal pads, elastic pressure bandages, and a rubber tourniquet as a last-resort option. Its field manual follows a five-phase deployment protocol: scene assessment, massive hemorrhage control, secondary casualty and airway management, wound management and shock prevention, and structured handoff to EMS. Responders should note that the included tourniquet is not CoTCCC-recommended and should be augmented with a windlass-style device such as a C-A-T or SOFTT-W for reliable field performance.

Critical limitations are documented directly in the SOP and govern safe deployment. The kit does not include occlusive chest seals, making it inadequate as a standalone solution for penetrating chest trauma. The suture set must not be used by non-licensed personnel, and the oral airway device requires specific training and is indicated only for deeply unconscious casualties with no gag reflex. Two included glove pairs are insufficient for multi-casualty events.

Readiness sustainment requires a monthly inspection cycle covering full inventory, expiration dates, packaging integrity, and bag condition. Any item that is expired, compromised, or consumed must be replaced before the kit returns to service. Recommended augmentations include CoTCCC-approved tourniquets, vented chest seals, additional nitrile gloves, a nasopharyngeal airway, and an emergency blanket.

Field Application Steps

1. CONFIRM SCENE SAFETY: Ensure the immediate threat is neutralized before approaching casualties. Do not advance until the scene is declared safe and you cannot become a casualty yourself.

2. DON PPE: Open the kit and immediately don one pair of latex examination gloves before initiating any contact with a casualty. Direct a bystander to contact emergency services simultaneously.

3. TRIAGE CASUALTIES: Conduct a rapid assessment of all casualties present. Identify and prioritize the individual with life-threatening hemorrhage before addressing lesser injuries.

4. EXPOSE THE WOUND: Use the EMT shears to cut away clothing and gear covering the injury site. Full visualization of the wound source is required before applying any dressing.

5. APPLY DIRECT PRESSURE: Place a sterile ABD pad (5"x9") directly onto the wound and apply firm, sustained manual pressure. This is the first intervention for all significant bleeding.

6. ESCALATE HEMORRHAGE CONTROL: If direct pressure does not control bleeding, pack the wound cavity with a BleedStop hemostatic bandage. Continue holding firm pressure over the packed wound.

7. APPLY PRESSURE DRESSING: Secure the ABD pad and hemostatic gauze in place by wrapping a 2" elastic bandage firmly over the wound site to maintain constant, direct pressure.

8. APPLY TOURNIQUET AS LAST RESORT: If bleeding remains uncontrolled on an extremity, apply the rubber tourniquet 2 to 3 inches above the wound, stretch tightly, and secure. Record and communicate the exact time of application to arriving EMS.

9. ASSESS SECONDARY CASUALTIES: Once the life-threatening hemorrhage is controlled, assess all remaining casualties for airway, breathing, and circulation. Use the OPA only if the casualty is deeply unconscious with no gag reflex and you have been trained in its use.

10. MANAGE MINOR WOUNDS: Control lacerations and abrasions using 4"x4" sterile gauze pads with direct pressure. Clean intact skin around wounds with antiseptic wipes and secure dressings with rolled gauze or adhesive tape.

11. PREVENT SHOCK: Keep all casualties warm and positioned flat with legs elevated if injuries permit. Do not administer pain relievers to unconscious casualties, those with suspected head injuries, or those who may require surgery.

12. EXECUTE HANDOFF TO EMS: Prepare a concise report for arriving paramedics including casualty count, injury types, all interventions performed, and tourniquet application time if applicable. Follow all directions from higher echelon providers.
